Impact of Sex on Comparative Outcomes of Bivalirudin versus Unfractionated Heparin in Patients with Acute Coronary Syndromes Undergoing Invasive Management A pre-specified analysis of the MATRIX trial.
CONCLUSIONS: In ACS patients, the rates of MACE and NACE were not significantly lower with bivalirudin than with UFH in both sexes.The rate of the composite of urgent TVR,definite ST,or NACE was not significantly lower with a post-PCI bivalirudin infusion than with no post-PCI infusion in both sexes. Minimally Invasive Cardiac Surgery: Removal of an Interatrial Intraseptal Bronchogenic Cyst Through a Periareolar Approach
A 58-year-old white male with a history of stroke and deep vein thrombosis presented with an interatrial intraseptal mass. Cardiac-computed tomography demonstrated a thin-walled, well-demarcated cyst in the inferior border of the fossa ovalis protruding into both atria. Removal of the interatrial intraseptal cyst was performed using a minimally invasive three-dimensional endoscopic periareolar approach.

Randomized Evaluation of a Novel Biodegradable Polymer-Based Sirolimus-Eluting Stent in ST-Segment Elevation Myocardial Infarction: The MASTER Study.

Clinical Leaflet Thrombosis in Transcatheter and Surgical Bioprosthetic Aortic Valves by 4DCT
ConclusionsIn patients with clinical or echocardiographic indications suggestive of leaflet thrombosis, 1/3rd of patients were found to have evidence of leaflet thrombosis using 4DCT. This allowed tailored anticoagulation therapy with resolution of the thrombus in most patients avoiding unnecessary anticoagulation in the remaining 2/3rds of patients.

In pursuit of the optimal hemocompatible left ventricular assist device
Left ventricular assist device (LVAD) implantation is a firmly established treatment modality for advanced heart failure; however, major complications such as nonsurgical bleeding, driveline infections, pump thrombosis, and stroke limit wider application to patients with less severe heart failure. Of note, more than 60% of patients who receive a continuous flow LVAD experience such severe adverse events within the first year.1

CYP2C19 pharmacogenetics versus standard of care dosing for selecting antiplatelet therapy in patients with coronary artery disease: A meta ‐analysis of randomized clinical trials
ConclusionIn patients undergoing stent implantation, MACE with genotype ‐guided therapy was not significantly reduced; however, there was a signal towards reduction of MACE in ACS patients, as well as a lower rate of MI, though this will require further confirmation in adequately powered trials.

Antithrombotic Strategies after Bioprosthetic Aortic Valve Replacement: A Systematic Review
ConclusionsTreatment with warfarin or aspirin both lead to similar outcomes after surgical bAVR. Combining aspirin with warfarin may lead to a small decrease in thromboembolism and mortality but is accompanied by increased bleeding. For TAVR patients, aspirin is equivalent to DAPT for reducing thromboembolism and mortality, with a possible decrease in bleeding.

A Novel Toroidal-Flow Left Ventricular Assist Device Minimizes Blood Trauma: Implications of Improved Ventricular Assist Device Hemocompatibility
ConclusionsThe TORVAD design, with markedly lower shear stress and pulsatile flow, caused significantly less blood trauma that the HeartMate II. LVADs with reduced blood trauma are likely to improve clinical outcomes and expand LVAD therapy into patients with less advanced heart failure.

Outcomes after Ross procedure in adult patients: A meta ‐analysis and microsimulation
ConclusionsRoss procedure in nonelderly adults is associated with low mortality and low risk of adverse events both at short ‐ and long‐term follow‐up. The surgical community must prioritize a large, expertize‐based randomized controlled trial to definitively address the risks and benefits of the Ross procedure compared to conventional aortic valve replacement.

Commentary: Postoperative atrial fibrillation can last years? Oh snap!
Despite improvements in perioperative and operative care, postoperative atrial fibrillation (POAF) is still found in approximately 15% to 60% of cardiac surgical patients.1 The potential clinical impact on patients is dependent on mainly their clinical status and can include heart failure, acute renal failure, stroke or thromboembolism, anticoagulation-related complications, and even death. This is particularly true because a significant proportion of patients with aortic stenosis have diastolic dysfunction and rely on the atrial contraction to maintain adequate cardiac output.

Final results of the Randomised Evaluation of short-term DUal antiplatelet therapy in patients with acute Coronary syndromE treated with a new generation stent (REDUCE) trial.
CONCLUSIONS: The results show that among ACS patients treated with COMBO, 3 months is non-inferior to 12 months DAPT. However, given the numerically higher rates of mortality and ST in the 3 months DAPT group, 1-year DAPT should still be recommended in ACS until more information becomes available.
