Inherited thrombophilia associated with ischemic pediatric stroke in parents-child pairs
We aimed to examine inherited thrombophilia frequencies by extending genetic profile to previously rarely or not-investigated polymorphisms, in children with ischemic pediatric stroke (IPS) and their parents. The study included 33 children: 23 with perinatal (PAIS), 8 with childhood arterial ischemic stroke (CAIS) and 2 with sinovenous thrombosis (SVT) and their parents (33 mother-child, 12 father-child and 12 mother-father-child pairs). Genotyping of FV-Leiden, FV-H1299R, FII-G20210A, β-fibrinogen-455G>A, FXIII-A-Val34Leu, PAI-1(4G/5G), HPA-1, MTHFR-C677T, MTHFR-A1298C, ACE(I/D) and APOE( ε2-4) was performed using CVD St...

Sex differences in acute telestroke care: more to the story
ConclusionWhile treatment differences between sexes existed in the data and were apparent in univariate analysis, no significant difference was seen in multivariate analysis once stroke risk factors, age, NIHSS score and admitting diagnosis were taken into consideration in the telestroke setting. Differences in rates of thrombolysis between sexes may therefore be reflective of differences in risk factors and symptomatology rather than a healthcare disparity.

Blood group and ischemic stroke, myocardial infarction, and peripheral vascular disease: A meta-analysis of over 145,000 cases and 2,000,000 controls
The ABO blood groups have been associated with the pathophysiology of cardiovascular diseases through an influence on two important blood clotting proteins, von Willebrand factor (vWF) and factor FVIII levels (FVIII).1,2 These proteins circulate in the plasma as a complex and are vital in normal haemostasis.1 The highest levels of FVIII and VWF are seen in non-O blood groups. Studies suggest a relationship between non-O blood groups and an increased risk of venous thromboembolism and other thrombotic episodes.

Multicenter Comparison of the Safety and Efficacy of Clopidogrel Versus Ticagrelor for Neuroendovascular Stents
ConclusionsUsing DAPT-C or DAPT-T regimens in a broad population of neuroendovascular stenting procedures appears to have similar safety and efficacy profiles. Further prospective evaluation is warranted to streamline the practice of DAPT selection and monitoring to determine the impact on clinical outcomes.

Neuro-Ophthalmic Visual Impairment in the Setting of COVID-19
We describe and elaborate on mechanisms of disease, including para-infectious inflammation, hypercoagulability, endothelial damage, and direct neurotropic viral invasion. Despite global vaccination programs, new variants of COVID-19 continue to pose an international threat, and patients with rare neuro-ophthalmic complications are likely to continue to present for care.Afferent complications from COVID-19 include homonymous visual field loss, with or without higher cortical visual syndromes, resulting from stroke, intracerebral hemorrhage, or posterior reversible leukoencephalopathy. Optic neuritis has frequently been repo...

Evolving Thrombolytics: from Alteplase to Tenecteplase
AbstractAlteplase has been the primary thrombolytic used in the treatment of acute ischemic stroke since thrombolysis was first established as an effective treatment of acute ischemic stroke in 1995. Tenecteplase, a genetically modified tissue plasminogen activator, has gained attention as an attractive alternative to alteplase given its practical workflow advantages and possible superior efficacy in large vessel recanalization. As more data is analyzed both from randomized trials and non-randomized patient registries, there is mounting support that tenecteplase appears to be at least equally, if not more, safe and potenti...

Intravenous thrombolysis of ischemic stroke-Current status
Nervenarzt. 2023 May 30. doi: 10.1007/s00115-023-01500-9. Online ahead of print.ABSTRACTIntravenous thrombolysis (IVT) treatment with alteplase (rtPA) is an essential part of the routine treatment of patients with ischemic stroke since its introduction in the late 1990s. Rapid treatment is of essential importance. For patients with an unclear time window, various mismatch concepts have been established to identify salvageable brain tissue prior to IVT. Numerous official contraindications for rtPA are not evidence-based; for example, current data from observational studies show that systemic thrombolytic treatment is possib...

An elderly case with late carotid stent thrombosis: possible role of antiphospholipid antibodies
The case study speculates that the antiphospholipid antibodies acquired during the follow-up period of carotid artery stenting may cause late stent thrombosis that is resistant to direct oral anticoagulants. A 73-year-old man was hospitalized with complaints of weakness in the right lower extremity. The patient had undergone carotid artery stenting for symptomatic stenosis of the left internal carotid artery 6 years prior and had received antiplatelet therapy with clopidogrel 75  mg/day. As the patient had developed atrial fibrillation without stent stenosis at the age of 70 years, anticoagulation therapy with rivaroxaban...

Functional and technical outcomes in acute ischemic stroke patients with hyperdense middle cerebral artery sign treated with endovascular thrombectomy
ConclusionPatients with HMCAS treated with EVT do not have a worse outcome at 3  months compared with no-HMCAS patients. Patients with HMCAS required a greater number of thrombus passes and longer procedure times.
