Pulmonary fat embolism of neoplastic origin
ABSTRACT Obstructive sleep apnea (OSA) is a common clinical condition, with a variable and underestimated prevalence. OSA is the main condition associated with secondary systemic arterial hypertension, as well as with atrial fibrillation, stroke, and coronary artery disease, greatly increasing cardiovascular morbidity and mortality. Treatment with continuous positive airway pressure is not tolerated by all OSA patients and is often not suitable in cases of mild OSA. Hence, alternative methods to treat OSA and its cardiovascular consequences are needed. In OSA patients, regular physical exercise has beneficial effects other...

The role of the Nox4-derived ROS-mediated RhoA/Rho kinase pathway in rat hypertension induced by chronic intermittent hypoxia
ConclusionHypertension can be induced by CIH in SD rats. The CIH-induced elevation of BP is at least partially mediated via the Nox4-induced ROS/RhoA/ROCK pathway.

Stratification of cardiovascular risk in patients with atrial fibrillation and obstructive sleep apnea —validity of the 2MACE score
ConclusionOSA prevalence is increased in AF patients and is associated with an increase 2MACE score —an indicator of major cardiovascular events. There is a linear relationship between severity of OSA and increasing 2MACE scores, indicating increasing cardiovascular risk related to OSA severity.

Idiopathic and Systemic Sclerosis associated Pulmonary Arterial Hypertension: A Comparison of Demographic, Haemodynamic and Magnetic Resonance Imaging Characteristics and Outcomes.
CONCLUSION: The reasons for poorer outcomes in SSc-PAH are likely to be multifactorial including, but not limited to, older age and reduced gas transfer. PMID: 28223154 [PubMed - as supplied by publisher]

Fine Particulate Air Pollution and Daily Mortality: A Nationwide Analysis in 272 Chinese Cities.
CONCLUSIONS: This nationwide investigation provided robust evidence of the associations between short-term exposure to PM2.5 and increased mortality from various cardiopulmonary diseases in China. The magnitude of associations was lower than those reported in Europe and North America. PMID: 28248546 [PubMed - as supplied by publisher]

Diagnosing and managing sleep apnea in patients with chronic cerebrovascular disease: a randomized trial of a home-based strategy
ConclusionsPatients with cerebrovascular disease and hypertension have a high prevalence of OSA. The use of portable polysomnography, and auto-titrating CPAP in the patients ’ homes, improved both the diagnosis and the treatment for sleep apnea compared with usual care but did not lower blood pressure.

Screening Programs for Obstructive Sleep Apnea
AbstractPurpose of the ReviewThe aim of this article is to describe the literature on the rationale, methods, and outcomes of screening programs for obstructive sleep apnea (OSA).Recent FindingsThe evidence does not support screening for OSA in the general population. Instead, targeted case finding is advised among the following high-risk groups: patients with obesity, congestive heart failure, atrial fibrillation, treatment refractory hypertension, type 2 diabetes, stroke, nocturnal dysrhythmias, and pulmonary hypertension. However, the method of screening remains unclear with some studies suggesting the use of questionna...

Do patients with obstructive sleep apnoea deserve new dedicated antihypertensive strategies?
Hypertension affects 25% of the adult population and remains a leading cause of cardiovascular mortality, accounting for 13.5% of all deaths. Half of all strokes and ischaemic heart disease events are attributed to hypertension.1 2 Most patients exhibit grade I hypertension (systolic blood pressure of 140–159 mm Hg and/or diastolic blood pressure of 90–99 mm Hg) without coexisting cardiovascular disease. Effective reduction of blood pressure in this population significantly reduces stroke and death.3 Obstructive sleep apnoea (OSA) is now recognised as a risk factor for the development of hyp...

Severe obstructive sleep apnea is associated with cochlear function impairment
ConclusionsAcoustic transference function of middle ear is similar in adults with and without OSA. Severe OSA is independently associated with cochlear function impairment in patients with no significant co-morbidities.

Features and Outcomes of Methamphetamine Associated Pulmonary Arterial Hypertension.
CONCLUSIONS: Meth-APAH is a severe and progressive form of PAH with poor outcomes. Future studies should focus on mechanisms of disease and potential therapeutic considerations. PMID: 28934596 [PubMed - as supplied by publisher]

Management of hypertension in obstructive sleep apnoea: predicting blood pressure reduction under continuous positive airway pressure
Hypertension affects a quarter of the adult population and remains a leading cause of cardiovascular mortality, accounting for 13.5% of all deaths. Half of all strokes and ischaemic heart disease events are attributable to high blood pressure (BP) [1, 2]. Most patients exhibit Grade-I hypertension (systolic BP of 140 to 159 mmHg and/or diastolic BP of 90 to 99 mmHg) without co-existing cardiovascular disease. An active reduction of BP results in a significant reduction in stroke and death [3].

Association of inflammatory biomarkers with comorbidity in chronic obstructive pulmonary disease - results from the COSYCONET cohort
Conclusion: WBC correlates with disease severity and is associated with relevant comorbidities. Other classical inflammatory biomarkers did not show a clear correlation with COPD severity of existing comorbidity.
