Association of formal and informal care with health-related quality of life and depressive symptoms: findings from the Caring for Adults Recovering from the Effects of Stroke study.
Conclusions: Shared care was the most common care configuration for stroke participants, but no significant associations were found between shared care and self-reported outcomes after adjusting for stroke participant characteristics. Further research is needed on the dose of informal and formal care and their coordination to better understand relationships with recovery. IMPLICATIONS FOR REHABILITATION Stroke is one of the leading causes of disability for adults in the USA and ongoing care is needed for survivors. Informal caregivers such as family members play an important role in recovery in addition to formal care prov...

Hospital Discharge Disposition of Stroke Patients in Tennessee.
CONCLUSIONS: Our investigation revealed that the hospital discharge disposition pattern of patients with stroke in Tennessee was associated with the key patient characteristics of selected demographics, clinical indicators, and insurance status. These analyses resulted in the development of an easy-to-use predictive tool for early determination of hospital discharge disposition status. PMID: 28863224 [PubMed - in process]

Severity of Traumatic Brain Injury in Older Adults and Risk of Ischemic Stroke and Depression
Conclusion: In this group of older adults with TBI, higher TBI severity was associated with increased risk of ischemic stroke, but not depression. These results suggest that increased monitoring of older adults with moderate-severe TBI for stroke may be warranted.

Factors associated with mental health service access among Australian community-dwelling survivors of stroke
CONCLUSIONS: Many stroke survivors have untreated depression/anxiety. Primary care practitioners should be supported in undertaking effective detection and management. Older and newly diagnosed individuals should be educated about depression/anxiety and available supports.Implications for rehabilitationPrimary care providers play a pivotal role in the pathway to mental health care, and therefore should always screen for depression/anxiety and provide comprehensive assessment and referral to specialist services where necessary.Targeted psychoeducation should be provided to survivors of stroke who are older and newly diagnos...

The Association of Depression, Cognitive Impairment Without Dementia, and Dementia With Risk of Ischemic Stroke: A Cohort Study
Conclusions: CIND and co-occurring depression and CIND are independently associated with increased risk of ischemic stroke. Individuals with co-occurring depression and CIND represent a high-risk group that may benefit from targeted interventions to prevent stroke.

The association of a heart attack or stroke with depressive symptoms stratified by the presence of a close social contact: findings from the National Health and Aging Trends Study Cohort
ConclusionsOlder adults have increased odds of having depressive symptoms following a self‐reported stroke, but only those with no close social contacts had increased odds of depressive symptoms following a heart attack. Social networks may play a role in the mechanisms underlying depression among older adults experiencing certain acute health events. Future work exploring the potential causal relationships suggested here, if confirmed, could inform interventions to alleviate or prevent depression among at risk older adults. E-191 Influence of socioeconomic factors on the development of post-stroke depression in aneurysmal subarachnoid hemorrhage survivors
ConclusionThe overall risk of developing a new depression disorder after aSAH was 16% in this retrospective study. The female sex was close to 4 times and substance use were six and half time more likely to develop new depressive symptoms; therefore, these risk factors should be considered when screening patients for depression after aSAH.Disclosures D. P-029 Impact of in-hospital delirium on outcomes of acute ischemic stroke
ConclusionsDelirium complicates ~3.5% of AIS admissions, and is associated with increased risk of in-hospital death, longer stay, higher charges and decrease in likelihood of home discharge.Disclosures D. Depression and Anxiety Screens as Predictors of 8-Year Incidence of Myocardial Infarction and Stroke in Primary Care Patients
We examined depression and anxiety screens, and their individual items, as predictors of incident hard CVD events, myocardial infarction, and stroke for 8 years in a diverse sample of 2041 older primary care patients initially free of CVD. At baseline, participants completed self-report depression and anxiety screens. Data regarding CVD events were obtained from an electronic medical record system and the Centers for Medicare and Medicaid Services analytic files. Results: During follow-up, 683 (33%) experienced a CVD event. Cox proportional hazards models—adjusted for demographic and CVD risk factors—revealed that a p...

Incidence of Poststroke Depression in Patients With Poststroke Dysphagia
CONCLUSIONS: Our results demonstrated that persons with poststroke dysphagia were as, or slightly more, likely to have PSD compared to the general stroke population, and to our knowledge, they establish the first reported incidence of PSD in Medicare patients with dysphagia after AIS. Future research is warranted to further explore the effects of PSD on poststroke dysphagia.PMID:35858266 | DOI:10.1044/2022_AJSLP-21-00346

Becoming A Hosehead: Sleeping My Way To Better Health
CPAP machines are in the news this month after the New England Journal of Medicine released a study casting doubt on their effectiveness in preventing heart problems. See here for an explanation of the study and a summary of reasons why it is likely not accurate. Regardless of that study, let me tell you why I have become an unexpected missionary for the wonders of the CPAP. For the past year, I've been wrestling with a diagnosis of sleep apnea. What have I learned, even while kicking, screaming and denying, through the entire testing and education process? That it is a real thing, that I really do have it, and that I fee...
