Are Patients Who Undergo THA for Infection at Higher Risk for 30-day Complications?
CONCLUSIONS: Patients undergoing revision THA for infection undergo reoperation and experience major complications more frequently in a 30-day episode of care than patients undergoing aseptic revision THA. Without risk adjustment to existing alternative payment and quality reporting models, providers may experience a disincentive to care for patients with infected THAs, who may face difficulties with access to care. LEVEL OF EVIDENCE: Level III, therapeutic study. PMID: 31268424 [PubMed - in process]

In-Hospital Complications following Arthrotomy versus Arthroscopy for Septic Knee Arthritis: A Cohort-Matched Comparison
J Knee Surg DOI: 10.1055/s-0039-1693450There is a paucity of literature comparing the relative merits of open arthrotomy versus arthroscopy for the surgical treatment of septic knee arthritis. The primary goal of this study is to compare the risk of perioperative complications between these two surgical techniques. To this end, 560 patients treated for septic arthritis of the native knee with arthroscopy were statistically matched 1:1 with 560 patients treated with open arthrotomy. The outcome measures included major complications, minor complications, mortality, inpatient hospital charges, and length of stay (LOS). Major ...

82. Bariatric surgery diminishes spinal symptoms in a morbidly obese population: a 2-year survivorship analysis of cervical and lumbar pathologies
Bariatric surgery for morbid obesity helps to address common comorbidity burdens, including decreasing rates of myocardial infarction and stroke. The increased mechanical stresses on the spine caused by morbid obesity predispose patients to various spinal pathologies and are concerning for spinal surgeons, with previous studies suggesting poorer outcomes than the general population. The effects of bariatric surgery on diminishing spinal complaints or symptoms have yet to be elucidated in the literature.

73. Tranexamic acid in patients undergoing adult spinal deformity surgery
Antifibrinolytic agents are used during ASD surgery to minimize blood loss and transfusion requirements. Tranexamic acid (TXA) reduces the risk of blood loss and transfusion after ASD surgery, but there persists concern for prothrombotic effects, myocardial infarction, stroke, and postoperative neurologic events including seizures.

Effects of Zoledronate on Cancer, Cardiac Events, and Mortality in Osteopenic Older Women
ABSTRACTWe recently showed that zoledronate prevented fractures in older women with osteopenia (hipT‐scores between −1.0 and −2.5). In addition to fewer fractures, this study also suggested that women randomized to zoledronate had fewer vascular events, a lower incidence of cancer, and a trend to lower mortality. The present analysis provides a more detailed presentation of the adverse event data from that study, a 6‐year, double‐blind trial of 2000 women aged>65  years recruited using electoral rolls. They were randomly assigned to receive four infusions of either zoledronate 5 mg or normal saline at 18...

Effect of clopidogrel in bone healing-experimental study in rabbits.
CONCLUSION: Our results indicate that maintenance of perioperative clopidogrel treatment does not negatively affect bone healing but rather promotes it. Further research is needed in order to find useful applications of this finding. PMID: 31908992 [PubMed]

Predictors of fatal and non-fatal cardiovascular events in ANCA-associated vasculitis: data from the Toronto CanVasc cohort
Conclusions: In this cohort of patients with granulomatosis with polyangiitis and eosinophilic granulomatosis with polyangiitis, both traditional and disease related risk factors were predictive of cardiovascular events. Further prospective studies should elucidate the impact of these and other modifiable risk factors on cardiovascular risk in ANCA-associated vasculitis.

Heparin-Induced Thrombocytopenia after Total Knee Replacement.
Authors: Salášek M, Šlechtová J, Pavelk T Abstract Heparin-induced thrombocytopenia is a rare complication of treatment with both unfractionated heparin (UFH) and low molecular weight heparin (LMWH). Antibodies against the complex heparin-platelet factor 4 are the main cause of pathogenesis, resulting in the activation of thrombocytes, coagulation, endothelium, monocytes, neutrophils and subsequent highly prothrombotic state. The prothrombotic state can result not only in venous but also in arterial thrombosis at different locations (which is manifested apart from venous thromboembolic disease also by acute lim...

Increased Prevalence, Complications, and Costs of Smokers Undergoing Total Knee Arthroplasty
J Knee Surg DOI: 10.1055/s-0040-1713128Given a national push toward bundled payment models, the purpose of this study was to examine the prevalence as well as the effect of smoking on early inpatient complications and cost following elective total knee arthroplasty (TKA) in the United States across multiple years. Using the nationwide inpatient sample, all primary elective TKA admissions were identified from 2012 to 2014. Patients were stratified by smoking status through a secondary diagnosis of “tobacco use disorder.” Patient characteristics as well as prevalence, costs, and incidence of complications were compared. ...

Cardiovascular safety of denosumab across multiple indications: a systematic review and meta ‐analysis of randomized trials
AbstractThe cardiovascular safety of denosumab has not yet been evaluated in a systematic review. This systematic review and meta ‐analysis sought to quantify the number of randomized controlled trials (RCTs) of denosumab (against comparators) reporting cardiovascular adverse events (CAEs) and examine the balance of CAEs between treatment arms. MEDLINE, Embase andclinicaltrials.gov were searched from inception to 26th October 2019 for RCTs of denosumab versus comparators for any indication. Included trials were randomized, enrolled ≥100 participants and reported bone‐related outcomes. Primary outcome for analysis was...

Mortality and Morbidity of Surgical Management of Geriatric Ankle Fractures.
DISCUSSION: After controlling for comorbidities (ie, the ASA class), no increased risk is observed for the 30-day mortality or complication rate between geriatric ankle fracture in the 65 to 79 years old and the 80 to 99 years old age groups. LEVEL OF EVIDENCE: Prognostic level III, retrospective study. PMID: 32769723 [PubMed - in process]
