UK women's life expectancy 'second worst' in Western Europe
"British women have second worst life expectancy in Europe," The Guardian reports. This is one of the findings of a Europe-wide health report carried out by the World Health Organization (WHO). The report also warned that European levels of alcohol consumption, smoking and obesity are alarmingly high, which could result in the following possibility: "Young Europeans may die at an earlier age than their grandparents". In the interests of accuracy, we should point out that the claim British women have the second worst life expectancy in Europe is incorrect. This figure is based on an analysis of countrie...

Risk of cardiovascular events following COPD hospitalisation: Findings from the China Kadoorie Biobank cohort study
Conclusion: Among adult Chinese, AECOPD leading to hospitalisation was independently associated with an increased risk of subsequent incident CVD events, especially in the first 28 days post-COPD hospitalisation.(Funded by GSK).

LATE-BREAKING ABSTRACT: Study to understand mortality and morbidIty in COPD (SUMMIT)
Chronic obstructive pulmonary disease (COPD) is recognized as having significant systemic manifestations associated with increased mortality and morbidity (Agusti, Proc Am Thorac Soc 2005;2:367-70), in particular cardiovascular (CV) disease. More patients with moderate COPD die from lung cancer and CV diseases than from COPD (Calverley, N Engl J Med 2007;356:775-89).Post-hoc analyses from the TORCH study suggest an inhaled corticosteroid/long acting beta2 agonist (ICS/LABA) combination may reduce mortality and CV events (e.g. acute coronary syndrome [ACS] and stroke) in patients with moderate COPD (≥50% predicted FEV1) ...

Acute exacerbation in COPD: we must do more
If one asks medical students which emergencies they hold as especially important in internal medicine, more than 90% first mention myocardial infarction and stroke. This is not surprising, because these diseases occupy the first places in the mortality statistics contained in the Global Burden of Disease report [1]. Nevertheless, respiratory failure is only described as rare, despite the fact that chronic obstructive pulmonary disease (COPD) is ranked fifth and pneumonia seventh in the same report. While the high hospital mortality rate of myocardial infarction and stroke is well known, the risk of acute exacerbation of CO...

The problem of atrial fibrillation in patients with chronic kidney disease.
Abstract Chronic kidney disease (CKD) is associated with the risk of multiple life-threatening complications such as: progression to chronic renal failure and cardiovascular disease including coronary heart disease, heart failure and peripheral arterial disease. Also, atrial fibrillation (AF) is common in this group of patients. Factors contributing to the occurrence of AF in patients undergoing dialysis include: age, presence of coronary heart disease, echocardiographic abnormalities (low ejection fraction, atrial enlargement, valvular calcification, left ventricular hypertrophy), heart failure, chronic obstructi...

A cluster randomized controlled trial comparing relative effectiveness of two licensed influenza vaccines in US nursing homes: Design and rationale.
CONCLUSIONS: This high-dose influenza vaccine trial uniquely offers a paradigm for future studies of clinical and programmatic interventions within the framework of efforts designed to test the impact of changes in usual treatment practices adopted by health care systems. Prevalence of chronic diseases among older patients in German general practices.
CONCLUSIONS: Our study showed that the prevalence of CDs is high in the German elderly population. Hypertension was the most frequent chronic condition and around 25% of patients displayed at least four CDs. PMID: 26977142 [PubMed - in process]

Prediction of atrial fibrillation development and progression: Current perspectives.
Abstract Atrial fibrillation (AF) is the most common arrhythmia in clinical practice. Several conventional and novel predictors of AF development and progression (from paroxysmal to persistent and permanent types) have been reported. The most important predictor of AF progression is possibly the arrhythmia itself. The electrical, mechanical and structural remodeling determines the perpetuation of AF and the progression from paroxysmal to persistent and permanent forms. Common clinical scores such as the hypertension, age ≥ 75 years, transient ischemic attack or stroke, chronic obstructive pulmonary disease, and ...

The role of HATCH score in predicting the success rate of sinus rhythm following electrical cardioversion of atrial fibrillation.
CONCLUSIONS: HATCH score is useful in predicting short-term success of EC at early stages for patients with AF for whom the use of a rhythm-control strategy is planned. PMID: 27040017 [PubMed - as supplied by publisher]

Risk factors for post-operative respiratory failure among 94,621 neurosurgical patients from 2006 to 2013: a NSQIP analysis
ConclusionsBased on the NSQIP database, risk factors for respiratory failure after neurosurgery include pre-operative ventilator dependence, alcohol use, functional dependence prior to surgery, stroke, and recent operation. The overall rate of respiratory failure decreased from 4.1  % in 2006 to 2.1 % in 2013 according to these data.

Long-acting bronchodilator initiation in COPD and the risk of adverse cardio-pulmonary events: A population-based comparative safety study.
CONCLUSION: COPD treatment initiation with tiotropium compared with LABA does not increase cardiovascular risk in the first year of treatment. The risk of pneumonia is higher with LABA, a likely effect of the inhaled corticosteroids present in many LABA inhalers used in real world clinical practice.
