Providing End-of-Life Care in the Community: What Are the Challenges in Malaysia?
J Hosp Palliat Care. 2022 Sep 1;25(3):133-137. doi: 10.14475/jhpc.2022.25.3.133.ABSTRACTA 72-year-old woman with metastatic lung cancer to bone and brain and with left external iliac vein thrombosis was under the care of a community palliative care provider. She experienced an acute pain crisis due to acute limb ischemia of the left lower limb. Goals-of-care discussions were held with the patient and her family; she prioritized symptom control and end-of-life care at home. The family and patient were aware of her short prognosis. Her complex pain was managed by the community palliative team, and her family was empowered to...

Prognosis of breast cancer patients with brain metastasis treated with radiotherapy
CONCLUSION: The number of metastatic BC patients developing BM is high, and survival following local treatment remains poor. Several prognostic factors appear to influence survival after radiotherapy. Treatment of BC patients with BM should be individualized according to performance status, leptomeningeal disease, number of BM, and receptor status of the disease.PMID:37498539 | DOI:10.1080/0284186X.2023.2238551

The place of the relative at the time of the announcement of cancer progression: BABEL - a mixed-methods study
This study aims to explore the place of the relative in these triadic consultations and how this influences communication.METHODS: A mixed-methods research strategy was used. Triadic consultations for the announcement of cancer progression were recorded and following the 3 participants completed questionnaires comprising mirror-items. Recordings and answers were further investigated in a few semi-structured interviews. Comparison of quantitative responses (questionnaires) used Wilcoxon's test for matched series. Qualitative analyses (consultations, interviews) used grounded theory. Patients were over 18, followed for cance...

Use of Bevacizumab in recurrent glioblastoma: a scoping review and evidence map
CONCLUSIONS: Although OS benefits from BEV-containing regimens could not be verified in this scoping review, the PFS benefits and side effects control supported BEV application in rGBM. Combining BEV with novel treatments like tumor-treating field (TTF) and administration at first recurrence may optimize the therapeutic efficacy. rGBM with a low apparent diffusion coefficient (ADCL), large tumor burden, or IDH mutation is more likely to benefit from BEV treatment. High-quality studies are warranted to explore the combination modality and identify BEV-response subpopulations to maximize benefits.PMID:37316802 | DOI:10.1186/s12885-023-11043-6

Primary dedifferentiated chondrosarcoma of the lung with a 4-year history of breast cancer: A case report
CONCLUSION: This case revealed a rare primary DDCS of the lung with a medical history of breast cancer, meaning a worse prognosis and making it more difficult to treat.PMID:37215415 | PMC:PMC10198077 | DOI:10.12998/wjcc.v11.i13.3022

Clinical Factors Associated With 30-Day Mortality Among Patients Undergoing Radiation Therapy for Brain Metastases
CONCLUSIONS: Multiple factors including a lower KPS, progressive intrathoracic disease, progressive liver and/or adrenal metastases, and inpatient status were associated with 30-DM after RT. A higher KPS, brain metastases at initial diagnosis, and outpatient palliative care utilization were associated with survival beyond 30 days. These data may aid in identifying which patients may benefit from brain metastasis-directed RT.PMID:37152484 | PMC:PMC10157109 | DOI:10.1016/j.adro.2023.101211

What neurologists can do for neuro-oncology patients
Pract Neurol. 2023 Apr 5:pn-2022-003665. doi: 10.1136/pn-2022-003665. Online ahead of print.ABSTRACTNeuro-oncology is a branch of medical science concerned with managing central nervous system tumours and neurological complications of cancer. Patients with brain tumours need a multidisciplinary approach to their care and neurologists can play a key part within that team. This review shows how neurologists can contribute to the care of patients with neuro-oncological disease at various points during the illness, including at initial diagnosis, during symptom management and at end of life assisting with palliative seizure ma...

Radiosensitisation by olaparib through focused ultrasound delivery in a diffuse midline glioma model
CONCLUSIONS: Olaparib effectively radiosensitises DMG cells in vitro and reduces primary tumour growth in vivo when combined with RT. Further studies are needed to investigate the therapeutic benefit of olaparib in suitable preclinical PDX models.PMID:37019285 | DOI:10.1016/j.jconrel.2023.03.058

Single-Session Gamma Knife Radiosurgery for Patients With 20 or More Brain Metastases
CONCLUSION: Single-session SRS is a well-tolerated palliative treatment option even in patients with ≥20 brain metastases, achieving local control rate >90% with low risks of neurotoxicity while continuing concurrent systemic oncological care.PMID:37018427 | DOI:10.1227/neu.0000000000002482
