Reviews of Educational Material
Possession of the diploma of Fellow of the Royal College of Anaesthetists (FRCA) is an essential requirement for trainees in anesthesia in the United Kingdom. The final FRCA examination is typically attempted midway through the current 5-yr United Kingdom training program (postgraduate year 7), and it represents the gateway to higher specialist training in anesthesia. Specialist perioperative allergy clinic services in the UK 2016: Results from the Royal College of Anaesthetists Sixth National Audit Project
Conclusions and Clinical RelevanceDiagnostic testing is not harmonized, with marked variability in the NMBA panels used to identify safe alternatives. Chlorhexidine and latex are not part of routine testing in many centres.Poor access to services and patient information provision require attention. Harmonization of diagnostic approach is desirable, particularly with regard to a minimum NMBA panel for identification of safe alternatives. (Source: Clinical and Experimental Allergy)

Benchmarking against the National Emergency Laparotomy Audit recommendations
ConclusionMortality in patients following emergency laparotomy at Logan Hospital compares favourably with 11.1% reported by NELA. This may be partly attributable to case mix distribution as for each P‐POSSUM risk Logan Hospital mortality was at the upper end of that reported by NELA. Further Australia data are required. Improved compliance with NELA recommendations may improve outcomes. (Source: ANZ Journal of Surgery)

Evaluation of the Genesis Epidural-Spinal Injection Simulator
Teaching advanced technical skills to novice anaesthetists is challenging, particularly when there is a risk of significant complications.1 Equally, learning advanced technical skills, by practicing on patients, can be stressful for trainees and may impair performance.2 Simulation is increasingly used in medicine,3 –5 is recognised by the Royal College of Anaesthetists in their curriculum6 and may play a role in addressing these issues. Training anesthesiologists in out-of-operating room anesthesia
Purpose of review: The purpose of this review is to describe recent developments and current trends in training anesthesiologists in out-of-operating room anesthesia (OORA). Recent findings: In the United States, the Accreditation Council for Graduate Medical Education recently updated its training requirements to include a mandatory 2-week rotation in OORA for anesthesiology residents. This likely reflects the continuing expansion of anesthesia services in the out-of-operating room (OOR) environment as well as the increasing complexity of OOR procedures and medical acuity of patients in these settings. In the United King...

Topics for the national audit projects of the Royal College of Anaesthetists
Editor —Moppett and colleagues1 provide a very fair assessment of the value of the national audit projects (NAPs) performed in the UK during the last decade. However, I would question their statement that the choice of topics for the third and fourth projects performed by the Royal College of Anesthetists (NAP3 and NAP4) was ‘in part serendipitous, as a result of the coincidence of the desires of the specialist societies (Pain and Difficult Airway) and the Royal College of Anaesthetists’. The subjects for both audits were discussed at meetings of College Council and, as a member of that group at the time, I remember ...

Underestimating the size of the problem? UK anaesthesia for patients with obesity
Publication date: Available online 16 June 2017 Source:Obesity Research & Clinical Practice Author(s): James Palmer, Mike Sury, Claire Nightingale, Jaideep Pandit, Tim Cook We analysed data from 14,000 patients in the 2013 United Kingdom Anaesthesia Activity Survey of the Fifth National Audit Project of the Royal College of Anaesthetists and Association of Anaesthetists of Great Britain and Ireland to examine anaesthetic, organisational and clinical practice for patients with obesity. We compared practice then with current guidance on management of this patient group. Obesity was reported in 22%, mainly Blac...

An analysis of the delivery of anaesthetic training sessions in the United Kingdom
Summary We analysed data from the electronic rota system CLWRota, covering 2,689,962 anaesthetic sessions between 01/01/2014 and 31/12/2015, in 91 UK Trusts, in order to investigate trainees’ supervision. There were 8209 trainee attachments analysed, during which 618,695 sessions were undertaken by trainees. The number of supervised sessions per week that trainees worked varied considerably (median (IQR [range]) 2.6 (1.6–3.6 [0–10]) for all grades combined), with senior trainees more likely than junior trainees to be supervised for fewer than the three sessions per week mandated by the Royal College of Anaesthetists....

Fundamentals of Anaesthesia, 4th Edition.
Now in its fourth edition,Fundamentals of Anaesthesia is, and will remain, a classic of British anesthetic literature. First appearing in 1999, following the development of the syllabus of the feared Fellowship of the Royal College of Anaesthetists exam, the book aimed to cover the majority of the syllabus. Due to the ever-evolving nature of medical practice, not least with the increasing role of technology in anesthetic practice, whether in the form of ultrasonography or hemodynamic monitoring, this textbook has similarly evolved while remaining focused on the basic principles of anesthesia. Opportunities for system level improvement in antibiotic use across the surgical pathway
Optimizing antibiotic prescribing across the surgical pathway (before, during, and after surgery) is a key aspect of tackling important drivers of antimicrobial resistance (AMR) and simultaneously decreasing the burden of infection at the global level. In the UK alone, 10 million patients undergo surgery every year (The Royal College of Anaesthetists, 2015), which is equivalent to 60% of the annual hospital admissions having a surgical intervention (Anon, 2016). The overwhelming majority of surgical procedures require effectively limited delivery of antibiotic prophylaxis to prevent infections (Boucher et al., 2009; Bratzl...

25 years: looking back to going forward
<span class="paragraphSection">2017 is a momentous year for the Royal College of Anaesthetists (RCoA) as we celebrate the 25th Anniversary of receiving our Royal Charter. The past quarter of a century has seen anaesthesia make tremendous advances, many of which were first documented in the pages of the British Journal of Anaesthesia (BJA). Interacting with more than two-thirds of hospital in-patients,<a href="#aex027-B1" class="reflinks">1</a> anaesthesia is now the UK’s largest single secondary care speciality supported by a College, that has quadrupled in ...

The Primary FRCA Structured Oral Examination Study Guide.
The primary Fellow of the Royal College of Anaesthetists (FRCA) examination is taken early in anesthesiology training in the United Kingdom. This examination sets a high standard and covers a great breadth of scientific and medical knowledge, which is matched by this excellent study guide. The two books are divided into five areas: physiology, physics, pharmacology, special patient groups, and critical incidents. These match the primary FRCA syllabus well, with the exception of statistics. This is a sensible omission because the statistical knowledge required is general and may easily be found elsewhere. The content is tai...

Anaesthetic charts in acute icu admissions: development for use in the emergency department
Conclusion We revealed poor record keeping during critical care involvement in the emergency department and identified the need for a more reproducible and standardised method of data recording. An adapted version of a current anaesthetic chart has been found to be effective in increasing the quality and quantity of information recorded for emergency admissions. This improves adherence to the required standards, and is also recognisable and user friendly for staff conditioned to use the Trust's anaesthetic charts. (Source: Emergency Medicine Journal)
