5th National Audit Project (NAP5) on accidental awareness during general anaesthesia: patient experiences, human factors, sedation, consent, and medicolegal issues
The 5th National Audit Project (NAP5) of the Royal College of Anaesthetists and the Association of Anaesthetists of Great Britain and Ireland into accidental awareness during general anaesthesia (AAGA) yielded data related to psychological aspects from the patient, and the anaesthetist, perspectives; patients' experiences ranged from isolated auditory or tactile sensations to complete awareness. A striking finding was that 75% of experiences were for <5 min, yet 51% of patients [95% confidence interval (CI) 43–60%] experienced distress and 41% (95% CI 33–50%) suffered longer term adverse effect. Distress and...

Who operates when, where and on whom? A survey of anaesthetic‐surgical activity in Ireland as denominator of NAP5
Summary The second phase of the 5th National Audit Project of the Royal College of Anaesthetists and the Association of Anaesthetists of Great Britain and Ireland, concerning accidental awareness during general anaesthesia, consisted of a survey of anaesthetic activity in Ireland. A network of consultant anaesthetists co‐ordinated data collection from the anaesthetic departments of 46 public and 20 independent hospitals over seven days. Data on patients' characteristics, anaesthetic techniques, staffing, and admission and discharge arrangements were collected on all cases for which anaesthetic care (general, regional or...

A national survey (NAP5‐Ireland baseline) to estimate an annual incidence of accidental awareness during general anaesthesia in Ireland
Summary As part of the 5th National Audit Project of the Royal College of Anaesthetists and the Association of Anaesthetists of Great Britain and Ireland concerning accidental awareness during general anaesthesia, we issued a questionnaire to every consultant anaesthetist in each of 46 public hospitals in Ireland, represented by 41 local co‐ordinators. The survey ascertained the number of new cases of accidental awareness becoming known to them for patients under their care or supervision for a calendar year, as well as their career experience. Consultants from all hospitals responded, with an individual response rate of...

Current UK practice of pediatric supraglottic airway devices – a survey of members of the Association of Paediatric Anaesthetists of Great Britain and Ireland
ConclusionPediatric anesthesiologists appear slow to embrace second‐generation SADs. The role of SADs in the management of difficult airways is widely accepted. Research currently has little influence over the choice of which SAD to use, which is more likely determined by personal choice and departmental preference. There is a risk that some SADs are unsafe. Human factors in the development of complications of airway management: preliminary evaluation of an interview tool
Summary The 4th National Audit Project of the Royal College of Anaesthetists and the Difficult Airway Society (NAP4) analysed reports of serious events arising from airway management during anaesthesia, intensive care and the emergency department. We conducted supplementary telephone interviews with 12 anaesthetists who had reported to NAP4, aiming to identify causal factors using a method based on the Human Factors Investigation Tool (HFIT). We identified contributing human factors in all cases (median [range] 4.5 [1–10] per case). The most frequent related to: situation awareness (failures to anticipate, wrong decision...

Audit of advanced airway management in UK Emergency Departments following the Fourth National Audit Project of the Royal College of Anaesthetists and Difficult Airway Society
The 4th National Audit Project of the Royal College of Anaesthetists and The Difficult Airway Society (NAP4)1 was published in March 2011, and included important recommendations for UK Emergency Departments (EDs). To coincide with the launch of NAP4, we audited current service availability and equipment provision for airway management in UK EDs against the recommendations made in the NAP4 report.2 A questionnaire was emailed to the designated emergency airway lead in 118 UK EDs. In all, 47 hospitals responded, representing 20% of all UK EDs. The large majority (82%) of advanced airway interventions in the ED are undertaken...

Complications of regional anaesthesia
Abstract: Complications of regional anaesthesia can be divided into those specific to central neuraxial blockade, those specific to peripheral nerve blockade, and those that pertain to both. Fortunately, severe complications – namely, spinal cord damage, vertebral cord haematoma and epidural abscess – are rare. Here we have given an overview of these complications, with reference to updated incidences available following the 3rd National Audit Project (NAP3) of the Royal College of Anaesthetists. A thorough knowledge of anatomy and pharmacology, and a meticulous, unhurried technique are essential to prevent such compli...

[Perspectives] Perceptions of pain
Pain relief has a long history. The question has occupied human consciousness probably for as long as we've been sentient. In truth, though, over the passing millennia, the ability to combat pain has only gained incremental ground. An exhibition at London's Science Museum, however, supported by the UK's Royal College of Anaesthetists, aims to inform visitors how cutting-edge research is now providing more answers than questions to the problem posed by the most visceral of sensations.
