Duplicate Therapy- SSRI+SSRI/SSRI+SNRI
How often do you encounter this combination working in retail and how do you resolve it? I have seen numerous patients that have prescriptions for escitalopram 10 mg + escitalopram 20 mg from the same prescriber, in addition to something like venlafaxine/duloxetine or fluoxetine and/or amitriptyline from another. To make it worse, most of these all are usually a (re)fill 05 and only have DUR comments that only contain 'dose ok' or 'patient on both' without providing any more information. SSRI/SNRI Cross-Taper, increased risk for serotonin syndrome?
Just wondering, because I came across some discussions on patients that were on Cymbalta/Effexor, and due to no efficacy, was decided to be switched to a SSRI. Also pain docs who want to switch a patient off SNRI... But as we know, at higher doses of Effexor (>100 mg), less serotonin reuptake, and more on the Norepi reuptake. So if you do a cross-taper, e.g, tapering at 225 mg Effexor, starting 20 mg Prozac, is there a heightened risk of serotonin syndrome? Should you first taper off the... Lexapro in kids?
So, I was seeing a 15 yo boy admitted for cellulitis to peds floor, in the consult service who was prescribed Lexapro by his outpatient physician for anxiety. We were consulted for evaluation of his anxiety during day time. Very tiny dose of 10 mg. His mom didn't get it for him yet because they were worried about side effects. I definitely feel he would benefit from the medication but I have not seen it used to much in younger kids. I would rather just go with Prozac for the longer half... Lexapro in kids? Is it illegal to reuse an "outdated" prescription?
So for the past year I've gone through trial and errors with my doctors trying out different prescription (ie. Paxi, Prozac, Klonopin, Xanax, etc.). About 8 months ago I asked my doctor to switch me from Klonopin back to Xanax (something I had taken previously) because I figured that worked better. I still have my Klonopin prescription bottle with some pills in it, if I were to finish off that bottle, would that be illegal? Is having CF a good enough reason to get a Medical Marijuana Card?
So because I have CF, is that a good enough reason for a doctor to get me a MM Card? Ive done a lot of study about this subject. And I believe an MM Card will benefit me greatly because I have CF. It will help me eat more, as well as help with other issues. I am at a good weight. But I can't eat nearly enough calories the CF doctors say I should be eating. They say I should be eating 4,000 calories a day if I'm working out, and 3,000 calories a day if I'm sedentary. I can't at all eat nearly that much! Like today I ate a ton of food, and I found out that I only ate 2400 calories today. Right now, I'm so full and earlier ...
