Etiological and functional characterization of mixed ventilatory impairment in Tunisian patients
Conclusion: MVI occurs infrequently and is more commonly caused by a mixture of pulmonary parenchymal and non-pulmonary disorders. COPD, ILD, lung cancer, tuberculosis and BD compose the majority of etiologies in our sample. (Source: European Respiratory Journal)

Eyes and the heart: what a clinician should know
The eye is prone to various forms of afflictions, either as a manifestation of primary ocular disease or part of systemic disease, including the cardiovascular system. A thorough cardiovascular examination should include a brief ocular assessment. Hypertension and diabetes, for example, would present with retinopathy and dyslipidaemia would present with corneal arcus. Multisystem autoimmune diseases, such as Graves’ disease, rheumatoid arthritis and sarcoidosis, would present with proptosis, episcleritis and scleritis, respectively. Myasthenia gravis, while primarily a neuromuscular disease, presents with fatigable p...

Outcomes After Extent I Thoracoabdominal Aortic Repair: Focus on Heritable Aortic Disease
CONCLUSIONS: Open extent I TAAA repair was effective in patients with HTAD, with low operative mortality and adverse event rates, better late survival, and excellent long-term durability, making a compelling argument for preferring open repair in these patients.PMID:37866646 | DOI:10.1016/j.athoracsur.2023.10.012 (Source: The Annals of Thoracic Surgery)

A novel pathogenic variant located just upstream of the C-terminal Ser423-X-Ser425 phosphorylation motif in SMAD3 causing Loeys-Dietz syndrome
CONCLUSIONS: Our results revealed the presence of TGF-β paradox in this case with the novel loss-of-function SMAD3 variant. The precise mechanism underlying the paradox is unknown, but further research is warranted to clarify the influence of the SMAD3 variant type and location on the LDS3 phenotype as well as the molecular mechanism leading to LDS3 aortopathy.PMID:37864304 | DOI:10.1002/mgg3.2257 (Source: Molecular Medicine)

Absent right superior vena cava and persistent left superior vena cava: An incidental finding
Ann Card Anaesth. 2023 Oct-Dec;26(4):433-434. doi: 10.4103/aca.aca_91_23.ABSTRACTA patient with Marfan syndrome undergoing Bentall operation was found to have an absent right superior vena cava and persistent left superior vena cava. The dilation of coronary sinus raised the suspicion of persistent left superior vena cava. The diagnosis was confirmed by agitated saline contrast echocardiography and computed tomography of the chest.PMID:37861579 | DOI:10.4103/aca.aca_91_23 (Source: Annals of Cardiac Anaesthesia)
