Safe medication to give for someone who may be bipolar vs MDD?
Hi, I am now starting outpatient in my third year and I have noticed that some of the patients I inherited are on SSRI's with a diagnosis of Bipolar Disorder. I was always under the impression that you avoid SSRI's without a mood stabilizer. My guess is these patients were probably Bipolar in the past but now they are being treated for depression as the Bipolar symptoms went into remission. I was wondering though, for discussion sake, are atypicals and Lithium generally safe for someone... Safe medication to give for someone who may be bipolar vs MDD? (Source: Student Doctor Network)

Mental illness delaying license
Hi everybody. I have bipolar disorder type 2. My last hospitalization was 8 years ago for a depressive episode. I haven't had a manic episode in 12 years. At the time I didn't have health insurance and wasn't being treated. I have been on medication since then and have not had any issues. Made it through med school just fine. I thought everything was fine until a few days ago when I received a notice that I would need to set up a meeting with my state's physician health program. I... MANIC DEPRESSION/PTSD, Dismissed from Medical School, Quitting medical school
Help all. I'd really like some advice. Or suggestions, mostly suggestions. The summer before starting a U.S. allopathic medical school, I had a nervous breakdown after four years of sexual abuse and was psychiatrically hospitalized for two months, then subsequently diagnosed with post traumatic stress disorder and manic depression (bipolar I). Only a couple months later, my father unexpectedly died shortly into the first week of medical school and needless to say, after my first semester I... Is this an example of bipolar?
A friend of mine just had his world flipped on him. His girlfriend of 4 years, with whom he has 2 children with, just left him for another man. Now, this is normal. Happens all the time. The part that struck me as odd was her back story and how she is acting now. She's been a recovery addict/addicted to Xanax for almost 8 years off and on. Now during their relationship he has told me several things that I always thought was addict behavior, such as lying, stealing, etc. Although, a couple... Is this an example of bipolar? How Would You Treat Bipolar Mania and Anxiety
Here's an interesting theoretical case running through my head: Middle-aged man with diagnosis of bipolar with acute mania and anxiety. No other comorbidities. No allergies. Normal labs. This is your first time seeing the patient. How would you treat pharmacologically? If using benzodiazepines, what would you give for maintenance for anxiety? Some possibilities from the top of my head: - treat bipolar only and hope anxiety goes away once mania subsides - give monotherapy that treats... Unorthodox Use of Antipsychotics in Bipolar Disorder
Two questions: 1. I was reading UptoDate (Bipolar disorder in pregnant women: Treatment of mania and hypomania) and saw the following: "The efficacy of haloperidol for treating mood elevated syndromes appears to be comparable to risperidone and olanzapine, and superior to quetiapine and lithium." None of the attendings I work with prescribe typical antipsychotics for mania. Do you have experience doing so in non-pregnant patients? What was the result? 2. Do you use long-acting... TON neurotomy: have you ever had to bury a 150cm cannulae?
First time I've needed to bury to the hub a 150mm RF cannulae for C2/3 TON radiofrequency neurotomy. I've done his lumbar spine in the past too and had to use 200mm cannulae, and since he's put on weight don't think I will be able to repeat the lumbar RF. Is there any cannulae made longer than 200mm? Maybe I could use a thoracool probe with a perpendicular to MB approach...should reach... Medicine, Sleep Deprivation, Peanuts & Prejudice: Do No Harm not apply to students?
I feel that my idealism all my life, and lonerism led me to believe the world was better than i thought. Then i got into a medical school and experienced a part of the dark side of humanity (i have bipolar 1 and have lived with it for nearly 8 years). The following is long, TL/DR: Do no harm only applies to certain people. 1) forced chronic sleep deprivation from roommate (leaving name out) shared single room), occurred first term here (January 2015), due to no other available housing and... Is bipolar with multiple cycles in a day (ultradian cycling) actually bipolar?
I've read some lay accounts from people who claim to have a sort of ultra-ultra-ultra rapid cycling (ultradian) version of bipolar I, where they reportedly cycle multiple times in single day, but I've also heard that these types of "cycles" can be better explained by emotional regulation deficits, such as those seen in borderline personality disorder, and thus aren't indicative of true bipolar disorder. A quick pubmed search reveals relatively little literature on ultradian bipolar cycling,... Is bipolar with multiple cycles in a day (ultradian cycling) actually bipolar? So Lithium or Depakote for Bipolar I?
Ok, so our institution rams down our throats that Lithium >>>>> Depakote. But as I talk to other attendings, they tell me that their residency says Depakote >>>> Lithium. So I setup a poll and want to know, 1) Your preferred mood stabilizer for acute mania? 2) Your preferred mood stabilizer for maintenance bipolar therapy? Any good papers/studies to read on Lithium vs. Depakote? (Source: Student Doctor Network)
