Ethicon Releases PROXISURE Advanced Laparoscopic Suturing Device
Ethicon, part of Johnson and Johnson, is releasing in the U.S. its new flagship laparoscopic suturing device, the PROXISURE. It features the use of curved needles that come inside of single-use cartridges which slip onto the tip of the main device. The tip itself moves in all kinds of ways, articulating 45° in both directions and rotating 360°, all using mechanical operation. The shaft through which the tip is manipulated works through a 12 mm trocar. Having a curved tip allows the PROXISURE to work well on flat tissues, the needle naturally extending into the tissue as it spins around the device’s tip. The device...

New Glue Derived from Mussels to Make Fetal Surgeries Safer
Fetal surgeries using laparoscopic tools have proven to be effective at treating a number of difficult conditions that would otherwise lead to tragic results. A major difficulty with these surgeries is that the amniotic sac is penetrated in order to reach the fetus. The sac is extremely fragile and once the instruments are removed the remaining hole is difficult to seal. A tear could occur and quickly propagate, leading to a serious emergency. A glue based on a mussel adhesive is strong enough to suspend a bag of shellfish. Credit: Diederik Balkenende Researchers at University of California, Berkeley have been working on d...

Politically Correct or Just Correct?
Recently the staid, establishment surgical journal,Annals of Surgery, attracted some undesirable attention regarding a Presidential Address to the European Surgical Society theypublishedin last month's issue. The transcribed speech---- "Modern Surgeon: Still a Master of His Trade or Just an OPerator of Medical Equipment? ---by Polish surgeon Marek Krawczyk MD was roundly vilified on Twitter and elsewhere for the alleged crime of only using male pronouns when referring to surgeons. Following the backlash,Annalstook the extraordinary step of retracting the entire piece. Their statement on the retraction is bel...

Computer Vision Algorithm Studies Laparoscopic Procedures to Understand What ’s Going On
Laparoscopic surgeries are often automatically recorded from the point of view of the endoscope’s lens. This is thanks to built-in recording equipment that accompanies many commercial endoscopic systems. What isn’t easy is reviewing all those hours of footage to find something that may be useful for training clinicians or that may be used to improve laparoscopy-related equipment. Now researchers at MIT have reported at the International Conference on Robotics and Automation in Singapore on a new video processing system that can, on its own, identify different stages of laparoscopic surgeries, potentially allow...

FDA Clears GORE SYNECOR Preperitoneal Biomaterial Hernia Repair Device
Gore received 510(k) clearance from the FDA to introduce its GORE SYNECOR Preperitoneal Biomaterial hernia repair device in the U.S. The material has both permanent and bioresorbable components and can be used in most hernia repair procedures, including for intraperitoneal and preperitoneal placement, and during traditional open, laparoscopic, or even robotic surgeries. It is designed to help treatment consist of only a single surgery and to help facilitate tissue in-growth following implantation. Here’s some details from Gore about the structure of GORE SYNECOR: A macroporous knit of dense, monofilament polytetra...

Residents performing surgery: Why can ’t we reach a middle ground?
A comparison of appendectomy outcomes for senior general surgeons and general surgery residents revealed no significant differences in early and late complication rates, use of diagnostic imaging, time from emergency department to operating room, incidence of complicated appendicitis, postop length of stay, and duration of post-op antibiotic treatment. The only parameter in which a significant difference was seen was that attending surgeons completed the procedure significantly faster by 9 minutes — 39.9 vs. 48.6 minutes, but this may have been influenced by the fact that attending surgeons used laparoscopic staplers...

Mandate
A few weeks ago I was awakened by a 2 a.m. call from the ED regarding a case of pneumoperitoneum. I barely recall the specifics of the conversation, but I vaguely remember snippets of phrases, words that light the fires and compel immediate action: "free air, tender all over, hypotension". I donned some old scrubs and quickly drove in to the hospital. The patient wasn't much older than me. He looked healthy, had a robust build. No other medical problems. But his vitals were perilously unstable. Heart rate 120's. Blood pressure 70 systolic despite several liters of fluid. The CT showed...

Chronic pain: The “invisible” disability
Sometime back in 2010, a good friend of mine from college who had since become a pediatrician posted a complaint on Facebook about “made up” health conditions. “Fibromyalgia, I’m looking at you,” she wrote. At this time, pain was more of an occasional visitor in my body rather than the permanent tenant it has since become. Still, I was offended on behalf of those patients with the disease. Fast forward to today and my life is all about pacing. This is because everything I do — cook, sleep, work, walk — takes time. This gradual approach to every aspect of my life is not about enlightenment or mindfulness. It i...

Iatrogenic infertility - when infertility doctors cause infertility !
Being infertile is bad enough, but it's a hundred times worse when the infertility is actually caused by your doctor. This is what we call Iatrogenic Infertility and it is of two types. One is permanent , where your fertility is damaged irreversibly because of something which the doctor does. The second is temporary, where your fertility takes a beating because of the medical intervention he advises.Permanent damage is usually because of surgical procedures. The three commonest offenders in this regard arehysteroscopic metroplasty;laparoscopic ovarian drilling;andlaparoscopic surgery for treatment of endometriosis.Let's lo...

What this physician learned from being an ailing patient
I felt a gust of sharp pain a couple of centimeters to the right of my belly button. It will subside, I thought. All pain subsides. It was an easy dictum to follow as a 39-year-old pediatric eye doc who last spent a night in the hospital as a patient in 1977. I’d undergone bilateral laparoscopic inguinal hernia repair ten days earlier, so I was accustomed to brief squalls of discomfort after certain maneuvers — in this case, 30 minutes on the elliptical machine. My adherence to post-operative instructions was incomplete, but I reasoned that after more than a week of smooth sailing, the chance of an untoward event w...

Minimally Invasive Surgery Congress 2017: A Quick Summary
Discussion also focused on using virtual reality and simulators to train increasing numbers of surgical trainees faster, better, and cheaper—with the issue brought sharply into focus in the context of the growing global shortage in the number of surgeons needed to perform even basic procedures. Both cautious and enthusiastic outlooks of increased technology use in surgery were debated by participants—some advocated for a conservative balancing of costs to benefits in the short-term, others for embracing the digital revolution enthusiastically. “Virtual surgeon” and popular futurist Shafi Ahmed perhaps put it best: ...

Ten Years In
My first day as an attending general surgeon in Cleveland, Ohio was August 7th, 2006. I saw one patient with a hernia in the office that day and then, around 430 pm, the call came in from the pediatric ER about a kid with abdominal pain. Some healthy 17 year old boy with obvious early appendicitis. I booked the case, tip-toed my way through the laparoscopic appendectomy uneventfully and went home feeling awful proud of myself. It was exactly how I envisioned a life as a general surgeon. I had been a confident 5th year resident. I hadn't done a fellowship. I had felt ready. I was read...

Top Healthcare Companies in Robotics
Robotics has the power to completely reshape the landscape of healthcare both in its structure and its operation. Here, you find my own list about the most relevant companies and start-ups on the healthcare robotics market. With robotics for the sustainability of healthcare As I outlined in my open letter to regulators, the long-term sustainability of healthcare systems could be solved by automation powered by digital health technologies, such as artificial intelligence, 3D-printing or robotics. The latter could take over monotonous work from healthcare workers, which would allow them to focus more on patients and to have ...
