This is the first in a series of  interviews I plan on publishing.  Dr Phillip M. Hornbostel, M.D., FACS, FASMBS is an accomplished bariatric surgeon in Missouri who has performed thousands of weight loss procedures over the years.  He is also the resident dean of the commentariat at the physician-only social media website Sermo.  The following represents a series of email exchanges he and I had over the holidays: Dr Hornbostel, tell me about your professional journey from general surgeon to an exclusively bariatric practice.  I finished general surgery residency in 1984 and immediately wen...

IVF success story for couple from Mumbai
We are a couple from Mumbai who were trying to become parents after 2 years of our marriage (2007). But when it did not happen ,  we went to a General Gynaecologist. Gynac tried couple of things and prescribed medicines , but still no success , even after 4 months of doing ultrasounds and blood tests reports, all of which were normal. The doctor then suggested laparoscopy to see whether the tubes were open or not. We wondered that if medical science has advanced so much, why is there a requirement of laparoscopy just to find out if the tubes are open or closed, as were reluctant to undergo surgery. We took a second op...

Why is IVF documentation so poor in India ?
I see lots of patients who have failed IVF cycles in other clinics . What breaks my heart is the poor quality of medical documentation they have with them . They are very poorly informed , and don't know the dose of injections used for super ovulation ; what the size of their follicles were ; how many eggs were retrieved;  or what the quality of their embryos was. This is very disheartening and disappointing for multiple reasons at multiple levels. Firstly, we can use this to revise and tweak the IVF treatment protocol , so that the patient has a better outcome for their next IVF cycle . If you don't have all this val...

New Academic Year Thoughts
The academic year is well underway and thoughts now turn to the candidates that will be filing applications for graduate nurse anesthetists programs.  For those that are thinking about or are applying this year here are a few tips from someone fresh in the mix. Dear David, Words cannot express the gratitude that I have for the invaluable shadow experience you presented me and all the words of encouragement you provided me.  Forgive my redundancy, but thank you, thank you, thank you, thank you and thank you some more.  My journey to become a CRNA started when I was just in my third semester of nursing school.  During my...

senior assistant
a good surgeon does not imply a good assistant. i personally don't like my assistant to be equally qualified with me. more qualified assistants can sometimes be a nightmare. one of my role models in the department of surgery was my registrar when i started there. he was just a very nice guy. he was in fact such a decent guy most people wondered what the hell he was doing studying surgery at all. he just didn't seem like the type. but no matter how good an individual he was, he still had to learn how to operate. the boss believed in teaching us to remove gallbladders the old fashioned way. therefore in his firm there was ...

Laparoscopic Surgery Not Better For Surgeons
While there are many benefits for patients of minimally invasive surgery, it is not at all ergonomic for the surgeon. I know some surgeons that argue that using the Davinci robot system is the safest way to avoid it. However, not all surgery will be done that way as that is a 2 million dollar device. In the meantime, the wrist can certainly be affected by the reptitive motion of laparoscopy.MoreMedicine TagHealth Tag Save This Page (Source: Doc To Doc)
