Lobectomy or Total Thyroidectomy —Where Is the Pendulum now for Differentiated Thyroid Cancer?
Thyroid surgery remains an essential treatment of thyroid cancer. The historical one-size-fits-all approach to differentiated (papillary and follicular) thyroid carcinoma of total thyroidectomy with central lymph node dissection has been shown to be overtreatment with associated risk of perioperative complications including nerve palsy and hypoparathyroidism. Furthermore, thyroid lobectomy may obviate life-long thyroid hormone replacement. Low-risk thyroid cancers have a low risk of recurrence and those that do recur can be salvaged with reoperation without compromising prognosis. Perioperative risk stratification for recu...

A Nod to the Nodes
Lymph node metastasis in thyroid cancer is common and associated with an increased risk of locoregional recurrence (LRR). Although therapeutic central neck dissection is well established, prophylactic central node dissection (pCND) for microscopic occult nodal involvement is controversial and recommendations are based on low-level evidence. The potential benefits of pCND such as reducing LRR and re-operation, refining staging, and improving surveillance are enthusiastically debated and the decision to perform pCND must be weighed up against the increased risks of complications. Surgical Endocrinology
This issue of the Surgical Oncology Clinics of North America focuses on surgical endocrinology. Surgery of the thyroid, parathyroid, adrenal, and endocrine pancreas has been performed for centuries as part of general surgery. Only in the twentieth century did surgeons begin to consider the endocrine system in a more holistic way, incorporating an understanding of underlying hormonal/chemical perturbations with anatomic organs. During the twentieth century, there were many surgical pioneers in endocrine surgery that addressed newly defined diseases and syndromes. (Source: Surgical Oncology Clinics of North America)

The Development of Investigator-Initiated Clinical Trials in Surgical Oncology
Investigator-initiated trials (IITs) are designed by principal investigators who identify important, unaddressed clinical gaps and opportunities to answer these questions through clinical trials. Surgical oncologists are poised to lead IITs due to their multidisciplinary clinical practice and substantial research background. The process of developing, organizing, and implementing IITs is multifaceted and involves important steps including (but not limited to) navigating regulatory requirements, obtaining funding, and meeting enrollment targets. Here, the authors explore the steps, methodology, and barriers of IIT developme...
