The BILAG-2004 index is associated with development of new damage in SLE
ConclusionsBILAG-2004 index is associated with new damage. BILAG-2004 LDA and BST persistent LDA can be considered as treatment targets. (Source: Rheumatology)

DNA from macrophages induces fibrosis and vasculopathy through POLR3A/STING/type I interferon axis in systemic sclerosis
ConclusionsSSc presented increased DNA leakage and STING pathway activation. DNA from macrophages induced type I IFN signature of fibroblasts and ECs through POLR3A/STING pathway. Blocking POLR3A/STING axis provides a new therapeutic target for SSc. (Source: Rheumatology)

Severe Libman –Sacks endocarditis complicating antiphospholipid syndrome: a retrospective analysis of 23 operated cases
ConclusionSevere HVD leading to surgery was strongly associated with thrombotic APS, especially arterial phenotypes. Half of the reported patients presented cerebral stroke complicating the HVD. Valvular surgery carried a significant risk of CAPS. (Source: Rheumatology)

Anti-phosphatidylserine/prothrombin antibodies and thrombosis associate positively with HLA-DRB1*13 and negatively with HLA-DRB1*03 in SLE
ConclusionsHLA-DRB1*13 confers risk for both aPS-PT and thrombotic events in lupus. The association betweenHLA-DRB1*13 and thrombosis is largely, but not totally, mediated through aPS-PT.HLA-DRB1*03 was negatively associated with aPL and positively with favourable lipid levels. Thus,HLA-DRB1*03 seems to identify a subgroup of SLE patients with reduced vascular risk. (Source: Rheumatology)

Improved outcomes in rheumatoid arthritis with obesity after a weight loss intervention: randomized trial
ConclusionObese RA patients on the diet intervention achieved weight loss. There were significant between group improvements for RAPID3, adiponectin and leptin levels, and positive trends for DAS28 and HAQ-DI. Longer-term, larger weight loss studies are needed to validate these findings, and will allow for further investigative work to improve the clinical management of obese RA patients.Trial registrationClinicalTrials.gov,https://clinicaltrials.gov, NCT02881307 (Source: Rheumatology)

Cancer occurrence after SLE: effects of medication-related factors, disease-related factors and survival from an observational study
ConclusionHigh dosage of CYC and disease-related factors (low SLEDAI-2K, less mucocutaneous and haematologic involvement) were related factors for cancer occurrence after SLE, while no survival difference was observed. (Source: Rheumatology)

Development and psychometric evaluation of the PMR-Impact Scale: a new patient reported outcome measure for polymyalgia rheumatica
ConclusionThe PMR-IS offers researchers a new way to assess patient-reported outcomes in clinical studies of PMR. It has been developed robustly, with patient input at every stage. It has good construct validity and test –retest reliability. Further work is needed to fully establish its responsiveness and interpretability parameters, and to assess its real-world clinical utility. (Source: Rheumatology)

Is time a healer? How quality of life changes over time reported by parents of children and young people with juvenile idiopathic arthritis
ConclusionChildren and young people with JIA have low HRQoL across domains compared with the reference population. This improves within 3  years of diagnosis, with the greatest improvement within the first year. Early developmentally appropriate clinical intervention is recommended to reduce both psychosocial and physical impact of JIA. The lower HRQoL scores of females require further investigation. Identification and functional characterization of imbalanced osteoarthritis-associated fibronectin splice variants
ConclusionWe show that full-lengthFN1 downregulation and concomitant relativeFN1-208 upregulation was unbeneficial for deposition of cartilage matrix, likely due to decreased availability of the classical RGD (Arg-Gly-Asp) integrin-binding site of fibronectin. (Source: Rheumatology)

Early onset is an indication of the severity of DADA2 disease
ConclusionEarly-onset DADA2 may be more severe. Early administration of TNF inhibitor can effectively reduce recurrence and quickly alleviate the disease. (Source: Rheumatology)

Strong site-specific association of pharyngeal cultures with the onset of psoriatic arthritis and psoriasis, regardless of pathogen
ConclusionThere is a strong site-specific association between pharyngeal culture samples and an increased risk of PsA and psoriasis, regardless of the pathogen. This may indicate that the site of infection, rather than the pathogen, is associated with increased risk. (Source: Rheumatology)

The effectiveness of corticosteroid injection versus night splints for carpal tunnel syndrome: 24-month follow-up of a randomized trial
ConclusionOver 24  months, surgical intervention rates were low in both groups, but less frequent in the NS group. While there were no differences in the clinical effectiveness of CSI and NS, initial treatment with CSI may not be cost-effective in the long-term compared with NS. (Source: Rheumatology)

Distal coronary artery abnormalities in Kawasaki disease: experience on CT coronary angiography in 176 children
ConclusionsCAAs can, at times, occur in distal segments in isolation and also in association with, or extension of, proximal CAAs. CTCA demonstrates CAAs in distal segments of coronary arteries, including branches, in a significant number of children with KD —these cannot be detected on TTE. CTCA may therefore be considered as a complimentary imaging modality in children with KD who have CAAs on TTE. (Source: Rheumatology)

The role of IL-23 in joint disease
AbstractThe myeloid cell-derived cytokine interleukin (IL-23) has been identified as an orchestrator of chronic inflammatory disease. IL-23 is instrumental in the activation of effector T cells and innate lymphoid cells, which in turn allow the mobilization of pro-inflammatory macrophages and neutrophils from the bone marrow and the initiation of inflammation in the joints, the gut and the skin. While the role of IL-23 in mounting chronic skin inflammation in the context of psoriasis is well established, increasing evidence suggests that also colitis and psoriatic arthritis are controlled by IL-23. Psoriasis, Crohn ’s di...
