Enhancing the evidence base for spiritually integrated psychotherapies: Progressing the paradigm of practice-based evidence.
Spiritually integrated approaches to psychological therapies enable practitioners to be responsive to clients’ religious and spiritual concerns and have been combined with most mainstream therapeutic approaches. As such, although evidence is required regarding their efficacy, with randomized controlled trials having a privileged status, a single research paradigm is unlikely to deliver both a robust and relevant evidence base for the psychological therapies, that is, embedded in routine practice. The research paradigm of practice-based evidence provides a complementary approach to trials methodology as carried out in rou...

“Where have you been in this world?” a qualitative study of clients’ curiosity about their psychotherapists.
Psychotherapy, Vol 59(4), Dec 2022, 572-583; doi:10.1037/pst0000451Curiosity is widely seen as a basic human drive, important to the development of relationships as well as to the process of change in psychotherapy. Less attention, however, has been directed toward examining curiosity in the client–therapist relationship. In particular, we lack a comprehensive understanding of what occurs for clients when they become curious about their therapists. As a result, we aimed to explore clients’ experiences of curiosity about their therapists. Using a consensual qualitative research approach, we analyzed data from ten curren...

Therapist emotion and emotional change with clients: Effects on perceived empathy and session quality.
This study introduces the concepts of directional correspondence (DC), where therapists’ and clients’ emotions move in a similar direction from pre to postsession, and absolute agreement (AA), where therapist and client become closer in emotions from pre to postsession, to examine congruence in emotional change in therapist and client from pre to postsession. In addition, clients’ perception of therapist empathy was investigated as a mediator between therapists’ emotion/therapist–client emotional congruence and session outcome. Sixty nine clients and 49 therapists participated in this study over the course of the...

Building strong therapeutic relationships quickly: The effect of the perceived working alliance on veterans’ intensive PTSD treatment outcomes.
Psychotherapy, Vol 59(3), Sep 2022, 470-480; doi:10.1037/pst0000447Intensive treatment programs (ITPs) are successful at reducing posttraumatic stress disorder (PTSD) and depression symptoms in veterans. However, the role of the working alliance in the context of ITPs is largely unexplored. The purpose of this study was to examine veteran-rated working alliance with their individual cognitive processing therapy (CPT) provider as a predictor of changes in PTSD and depression symptoms as well as negative posttrauma cognitions in two unique ITP formats. Data were collected from 128 veterans who completed a 2-week ITP, involvi...

Oppression is not “culture”: The need to center systemic and structural determinants to address anti-Black racism and racial trauma in psychotherapy.
Psychotherapy, Vol 60(1), Mar 2023, 76-85; doi:10.1037/pst0000446Psychology, and the United States more broadly, finds itself at yet another reckoning point with White supremacy and anti-Black racism. The American Psychological Association has even recently apologized for psychology’s role in upholding systemic racism and White supremacy, and articulated expectations for psychology’s role in dismantling systemic racism and White supremacy throughout psychology. Yet, the norms of White supremacy pervade our professional and individual consciousness, resulting in our radical movements toward a “culturally responsive ps...

Within-day sudden gains and generalized anxiety disorder psychotherapy outcome.
We examined the association between occurrence of SGs, SLs, and reversals and symptom change from pretreatment to posttreatment, and 6-month, and 1-year follow-up. Clients in ND (61.1%) were more likely to experience SGs than clients in CBT (20.0%) or AR (38.0%). 92.9% of SGs were followed by a reversal. Experiencing SGs was associated with less symptom change from pretreatment to follow-up in ND and not associated with outcome in CBT or AR. SLs and reversals were not associated with outcome. When measured at a within-day level, SGs are very likely to be reversed and may be associated with poorer long-term outcome in ND. W...
