Effects of Tumor Necrosis Factor-α on Podocyte Expression of Monocyte Chemoattractant Protein-1 and in Diabetic Nephropathy
Conclusion: TNF-α potently stimulates podocytes to produce MCP-1, utilizing the TNFR2 receptor and the NF-#x03BA;B and PI3K/Akt pathways. Both TNF-α and MCP-1 levels were increased in the urine of diabetic db/db mice, correlating with the severity of diabetic albuminuria.Nephron Extra 2015;5:1-18 (Source: Nephron Extra)

The Effect of Shiga Toxin on Weibel-Palade Bodies in Primary Human Endothelial Cells
Conclusion: Stx1 can contribute, via an effect on WPbs, to the exocytosis of WPbs in flow conditions in HUVECs and probably in GMVECs. This results in the release of VWF, suggesting an initiating role of the coagulation system in the pathogenesis.Nephron Extra 2014;4:101-107 (Source: Nephron Extra)

Improvements in Renal Replacement Therapy Practice Patterns in Estonia
Conclusion: An improvement in the data collection was noticed, and the analysis of CPI allows renal centres to assess and compare their practices with others. The collection and evaluation of CPI of RRT patients is an important improvement and significantly increases the awareness of nephrologists.Nephron Extra 2014;4:108-118 (Source: Nephron Extra)

Renal AA Amyloidosis in Patients with Type 2 Diabetes Mellitus
Conclusions: RAAA is prevalent in T2DM and is associated with worse CVD and renal disease, likely because RAAA is a marker of severe chronic inflammation.Nephron Extra 2014;4:119-126 (Source: Nephron Extra)

Urinary Alpha- and Pi-Glutathione S-Transferases in Adult Patients with Type 1 Diabetes
Background/Aims: Glutathione S-transferases (GSTs) are cytosolic enzymes excreted from renal tubules following tubular damage. α-GST primarily originates from proximal tubules, while π-GST from distal tubules and collecting ducts. We investigated if GST levels are associated with renal function in patients with type 1 diabetes. Methods: We conducted a cross-sectional study including 189 Caucasian patients with type 1 diabetes and 16 nondiabetic controls. α- and π-GST were measured by ELISA and reported as GST/urinary creatinine excretion (μg/mmol). Results: The subjects were 53 ± 14 years old, 66 (35%) were female an...

Metabolic Syndrome in IgA Glomerulonephritis
Conclusion: The number of subjects with MetS among IgAGN patients and the general population is equal in Finland. MetS does not seem to be an independent prognostic variable.Nephron Extra 2014;4:138-145 (Source: Nephron Extra)

Safe and Effective Use of Chronic Transdermal Estradiol for Life-Threatening Uremic Bleeding in a Patient with Coronary Artery Disease
We present a patient on long-term peritoneal dialysis with coronary disease who developed recurrent life-threatening bleeding episodes secondary to uremia, where treatment with transdermal estrogen was used safely and effectively for a 24-month period.Nephron Extra 2014;4:134-137 (Source: Nephron Extra)

Neph1 Is Reduced in Primary Focal Segmental Glomerulosclerosis, Minimal Change Nephrotic Syndrome, and Corresponding Experimental Animal Models of Adriamycin-Induced Nephropathy and Puromycin Aminonucleoside Nephrosis
Conclusion: With nephrin (but not Neph1) unchanged in FSGS, there might be a disruption of the complex and an involvement of Neph1 in its pathogenesis.Nephron Extra 2014;4:146-154 (Source: Nephron Extra)

Collapsing Focal Segmental Glomerulosclerosis Resulting from a Single Dose of Zoledronate
We describe the case of a 36-year-old African American female with metastatic breast cancer who presented with nephrotic-range proteinuria and acute kidney injury within 2 weeks of exposure to a single dose of zoledronate. The patient had a partial recovery of her renal function and showed improved proteinuria to a subnephrotic level after discontinuing zoledronate. In contrast to 2 prior reports of zoledronate-induced collapsing FSGS, the causative role of the exposure described here is certain. Our case necessitates the addition of zoledronate to the list of known causes of collapsing FSGS. Furthermore, it highlights the...

Frequent Hemodialysis Fistula Infectious Complications
Conclusions: The rate of buttonhole-related infections was high among patients on frequent hemodialysis and more than 50 times greater than that among patients on CIHD with the stepladder technique. Most bacteremias were due to S. aureus - with serious consequences. The risks and benefits of buttonhole cannulation require individual consideration with careful monitoring, prophylaxis and management.Nephron Extra 2014;4:159-167 (Source: Nephron Extra)
