Some patients (and some of us) respond better to some biological therapies: the as yet unsolved conundrum
AbstractVery often, treatment for many common musculoskeletal conditions is only palliative, or involves surgery with major shortcomings. Biological interventions —in particular, platelet-rich plasma (PRP) therapies—may well provide more effective treatments, but their actual efficacy is under scrutiny. PRP is biologically unique to each individual depending on endogenous and exogenous factors, including, but not limited to, demographic factors (i.e. age) , immune status (i.e. microbiota), metabolic diseases and concomitant medications. All these potential modifiers of the ultimate effects of PRP have been poorly explo...

Clinical and radiographic comparison of a single LP-PRP injection, a single hyaluronic acid injection and daily NSAID administration with a 52-week follow-up: a randomized controlled trial
ConclusionsLeukocyte-poor platelet-rich plasma (LP-PRP) injections are better in terms of clinical improvement with respect to HA injections or oral NSAID treatment in knee osteoarthritis patients at the 52-week follow-up. Moreover, a single LP-PRP injection is effective. However, LP-PRP has no influence on cartilage progression.Level of evidenceLevel II. (Source: Journal of Orthopaedics and Traumatology)

Percutaneous suction and irrigation for the treatment of recalcitrant pyogenic spondylodiscitis
ConclusionsOur study confirms the feasibility and safety of our percutaneous technique for irrigation and debridement of pyogenic spondylodiscitis. Percutaneous irrigation and suction offers a truly minimally invasive option for managing recalcitrant spondylodiscitis or for diagnostic purposes. The approach used is very similar to discography and can be easily adapted to different hospital settings.Level of EvidenceLevel III (Source: Journal of Orthopaedics and Traumatology)

The loading patterns of a short femoral stem in total hip arthroplasty: gait analysis at increasing walking speeds and inclines
ConclusionsIrrespective of femoral stem length, symmetry for ground reaction forces for both THA groups were returned to a normal range when compared to controls. However individual implant performance showed inferior (p <  0.05) push-off forces and normalised step length in both THR groups when compared to controls.Level of evidenceIII. Revision total knee arthroplasty: hybrid vs standard cemented fixation
ConclusionAlthough the results were similar for the two groups, hybrid fixation tended to produce better results than cemented fixation. In view of the risk of further loosening, we prefer the more conservative approach, i.e. hybrid fixation.Level evidenceLevel III. (Source: Journal of Orthopaedics and Traumatology)

Are clinical outcomes affected by type of plate used for management of mid-shaft clavicle fractures?
ConclusionAlthough different types of implants have different biomechanical properties, no difference in reoperation, union and plate removal rates were found between the various plate types. Future studies with a larger sample size are required to further examine these outcomes.Level of evidenceLevel III. (Source: Journal of Orthopaedics and Traumatology)

Assessing the real benefits of surgery for degenerative lumbar spinal stenosis without instability and spondylolisthesis: a single surgeon experience with a mean 8-year follow-up
ConclusionsAlthough the improvement of the first postoperative years decreases over time and despite the lack of general consensus, the lack of established shared guidelines and the limitations of this research, the results support the utilisation of surgery for the management of this condition.Level of Evidence3. Alleged malpractice in orthopaedics. Analysis of a series of medmal insurance claims
ConclusionsOrthopedics is a medical specialty with a high risk for malpractice claims. In our study, medical malpractice was observed in nearly 50% of the cases —typically in surgery-linked cases resulting in permanent impairment of the patient. Death from orthopedics malpractice seemed to be rare.Level of EvidenceIV. (Source: Journal of Orthopaedics and Traumatology)

Quantitative correlation of mechanoreceptors in tibial remnant of ruptured human anterior cruciate ligament with duration of injury and its significance: an immunohistochemistry-based observational study
ConclusionsNo correlation was found between duration of injury and presence of viable mechanoreceptors, hence it is beneficial to preserve the native ACL stump irrespective of the time interval between injury and surgery.Level of EvidenceIII. (Source: Journal of Orthopaedics and Traumatology)
