Long-term outcomes of distal locking in extracapsular fractures treated with trochanteric Gamma3 nails
ConclusionsAny type of distal locking seems to be safe for consolidation, despite a slightly longer consolidation time in static locking. Early cut-out was the main complication, while others were very infrequent, which is an advantage over helical blade devices. There was a higher rate of minor and overall mechanical complications in ST, but infection and lateral thigh pain were similar. Most non-traumatic mechanical complications occurred around 5 –6 weeks. About one in five of the DN truly dynamised, with all cases occurring before 8 weeks. Mobility until endpoint and overall long-term survival were not influenced b...

Problems, complications, and reinterventions in 4893 onlay humeral lateralized reverse shoulder arthroplasties, a systematic review: part II —problems and reinterventions
ConclusionsProblem, complication, and reintervention rates proved acceptable when implanting a high humeral lateralization stem RSA. The most frequent problem was scapular notching (12.6%), and the most common postoperative complication was scapular stress fracture (1.8%). An overall humeral complication rate of 1.9% was identified, whereas no humeral fractures or stem loosening were reported with short stems. Infections (1.3%) were the most common reason for component revision, followed by instability (0.8%).Level of evidenceSystematic review IV. (Source: Journal of Orthopaedics and Traumatology)

Even experts cannot agree on the optimal use of platelet-rich plasma in lateral elbow tendinopathy: an international Delphi study
ConclusionExperts reached very limited consensus on the use of PRP in LET. High levels of user experience have not resulted in a convergence of opinion on the technical components of PRP formulation and delivery, substantiating the need for further studies and improved trial reporting. Patient-reported results of simultaneous direct anterior approach and posterolateral approach total hip arthroplasties performed in the same patients
ConclusionsThis study demonstrates that DAA-THA and PLA-THA could provide comparable HHS and HOOS at all follow-ups. Compared with PLA-THA, DAA-THA is associated with less hip pain within postoperative 7  days and shorter incision length, but longer operative time.Level of evidenceLevel I, therapeutic study.Trial registration Chinese Clinical Trail Registry, ChiCTR1800019816. Endoscopic gluteus maximus tendon release for external snapping hip syndrome: a functional assessment
ConclusionsEndoscopic gluteus maximus tendon release is an excellent surgical option to treat snapping hip syndrome. The evaluated muscle strength revealed no functional impairment. The significance of the limb circumference reduction has yet to be determined.Level of evidenceIV: retrospective comparative trial. (Source: Journal of Orthopaedics and Traumatology)

Prosthetic joint infections and legal disputes: a threat to the future of prosthetic orthopedics
AbstractProsthetic joint infections (PJI) represent one of the major problems in orthopedic prosthetic surgery. The incidence of PJIs varies according to the site of intervention, and different published case studies report occurrence at 0.5 to 3.0% in the event of first implants, with a significant greater risk in the case of prosthesis revisions. The diagnosis of prosthetic infections is seldom simple, needing a multi-specialist approach, which includes the accurate collection of patient anamnesis, its clinical evaluation, the evaluation of inflammation biomarkers, and the use of imaging techniques. It is essential to id...

Analysis of predictors of mortality after surgical and non-surgical management in proximal humerus fractures
ConclusionsPatients with proximal humerus fractures and associated comorbidities (CCI>  5) presented higher mortality than healthier patients. This mortality risk was greater in patients with comorbidities if surgical treatment was indicated rather than conservative treatment. Patient’s comorbidities should be a fundamental parameter when planning the therapeutic strategy.Level of evidenceLevel 3. (Source: Journal of Orthopaedics and Traumatology)

Factors influencing knee valgus alignment in Crowe type IV hip dysplasia after total hip arthroplasty
ConclusionsThe postoperative mLDFA is a major factor related to knee valgus alignment after THA, which combines the preoperative anatomy and surgical reconstruction. Other factors previously published were found to have no significance.Level of evidenceIII. (Source: Journal of Orthopaedics and Traumatology)

Is intraoperative neuromonitoring effective in hip and pelvis orthopedic and trauma surgery? A systematic review
ConclusionsThe use of IONM during hip and pelvis surgery is debated. The review results are insufficient to support the routine use of IONM in hip and pelvis surgery. The different IONM techniques have peculiar advantages and disadvantages and differences in sensitivity and specificity without clear evidence of superiority for any. Results from different studies and different interventions are often in contrast. However, there is general agreement in recognizing a role for IONM to define the critical maneuvers, positions or pathologies that could lead to sciatic nerve intraoperative damage.Level of evidenceLevel 2. Return to physical activity and change in body mass index after hypoallergenic medial mobile-bearing unicompartmental knee arthroplasty
ConclusionsThis is the first study to evaluate the rate of the return to sports activities and change in BMI following hypoallergenic UKA. The majority of patients reduced their weight following UKA and improved their physical activity, showing outcomes that were comparable to the standard cobalt –chrome (CoCr) prostheses, regardless of gender, age, BMI and implant size.Level of evidenceIV – Prospective Cohort Study.Trial registration researchregistry5978 —Research Registrywww.researchregistry.com (Source: Journal of Orthopaedics and Traumatology)
