Surgeon-Anesthesiologist Dyad and Patients Undergoing Complex Gastrointestinal Cancer Surgery
To the Editor We read with great interest the article by Hallet et al, who used multivariable logistic regression to explore the association between familiarity of the surgeon-anesthesiologist dyad and postoperative patient outcomes for complex gastrointestinal cancer surgery and observed a linear association between dyad volume and 90-day major morbidity. The present study underscores that the increasing surgeon-anesthesiologist familiarity could improve the postoperative outcome. However, from the viewpoint of data interpretation and extrapolation, we have 2 concerns. (Source: JAMA Surgery)

Quality Trauma Care Does Not End at the Hospital ’s Lobby
In recent years, health care quality metrics have evolved from morbidity and mortality being key outcomes to evaluating long-term outcomes, patient-reported outcomes, and quality of life. For trauma, and specifically geriatric trauma, there has been little investigation into long-term outcomes as a measure of quality. As we move forward, and as our population ages, we are compelled to optimize trauma systems to obtain the best long-term outcomes for trauma survivors. (Source: JAMA Surgery)

Will Quantifying Fascial Tension Change the Practice of Component Separations in Complex Abdominal Wall Reconstruction?
Miller and colleagues published their work on the quantification of anterior and posterior fascial release after posterior component separation (PCS) and transversus abdominis release (TAR) using a proprietary, sterilizable tensiometer on anesthetized patients with ventral hernia. To our knowledge, this is the largest study of its kind in our review. Their findings demonstrated that anterior fascial tension was most impacted by the retrorectus dissection. This step contributed −82% of the total change in anterior fascial tension, compared with −18% from the incision of the internal oblique posterior lamella and 0% from...

Palliative Care for Patients Undergoing Surgery —Reply
In Reply We appreciate the thoughtful response to our recently published trial. We agree that the null findings stem from studying a population with low unmet palliative care needs. Prior randomized clinical trials have defined populations of patients with cancer who benefit from palliative care based on their diagnosis and treatment modality (eg, metastatic lung cancer treated with chemotherapy, hematologic malignancies treated with hematopoietic stem cell transplant). Our work along with the recently published work by Aslakson et al suggests that a finer-grained approach is needed to identify surgical patients with unmet...
