Supportive Care in Patients with Critical Coronavirus Disease 2019
Specific therapies for the treatment of coronavirus disease 2019 (COVID-19) have limited efficacy in the event a patient worsens clinically and requires admission to the intensive care unit (ICU). Thus, providing quality supportive care is essential to the overall management of patients with critical COVID-19. Patients with respiratory failure not requiring intubation should be supported with noninvasive positive pressure ventilation, continuous positive airway pressure, or high flow oxygenation. Use of these respiratory modalities may prevent patients from subsequently requiring intubation. Basic components of supportive ...

Supportive Care in Patients with Critical COVID-19
Specific therapies for the treatment COVID-19 have limited efficacy in the event a patient worsens clinically and requires admission to the intensive care unit (ICU). Thus, providing quality supportive care is essential to the overall management of patients with critical COVID-19. Intubated patients with COVID-19 should receive established ARDS supportive care that was established prior to the pandemic including low tidal volume ventilation and prone ventilation. Patients with respiratory failure not requiring intubation should be supported with noninvasive positive pressure ventilation, continuous positive airway pressure...

Management of Severe and Critical COVID-19 Infection with Immunotherapies
Early in the COVID-19 pandemic, clinicians and researchers sought to rapidly repurpose available candidate therapies for SARS-CoV-2 infection pending the development of directed antivirals and novel vaccines. Slowly, anecdotal case series and single-arm observational trials gave way to randomized control trials (RCTs) as the global research community mobilized to design, implement, and analyze studies in the midst of unprecedented pressure on healthcare systems. Despite early controversy surrounding the Emergency Use Authorization (EUA) and politicization of hydroxychloroquine therapy, progress soon followed in the form of...

Laboratory Diagnosis for SARS-CoV-2 Infection
The optimal diagnostic test for SARS-CoV-2 infection should be selected based on a patient ’s clinical syndrome and presentation in relation to symptom onset. Molecular testing, most often reverse-transcriptase polymerase chain reaction, offers the highest sensitivity and specificity during acute infection, whereas antigen testing can also be useful for acute diagnosis when rapid turnar ound of results is necessary or if molecular testing is unavailable. Serologic testing is often reserved for identifying individuals with prior or late COVID-19 infection. (Source: Infectious Diseases Clinics of North America)

COVID-19 in the Critically Ill Patient
The COVID-19 pandemic has led to significant mortality in the United States with over 800,000 deaths in 2020 and 2021. The proportion of patients with COVID-19 who develop severe disease varies but is decreasing over time with growing population immunity and improved therapeutic options. Patients who are ≥65 represent the largest proportion of deaths from COVID-19. Additional risk factors include immunosuppression and chronic medical conditions—such as diabetes mellitus, chronic kidney disease, and chronic heart disease. Vaccination dramatically reduces the risk of severe COVID-19.While critical illness from COVID-19 i...
