Circulatory support with venovenous ECMO in a patient with reperfusion pulmonary edema after pulmonary artery thromboendarterectomy
We present the case of a 60-year-old woman with a history of acute pulmonary embolism due to deep vein thrombosis of the right pelvic limb who was diagnosed later with CTEPH who was admitted for scheduled surgical treatment involving bilateral PTE. However, during the immediate postoperative period, she developed cardiogenic shock and refractory hypoxemia secondary to pulmonary reperfusion syndrome following the surgical procedure. As a result, she required veno-venous ECMO circulatory support for 6 days, leading to resolution of the pulmonary condition and clinical improvement.PMID:38359441 | DOI:10.24875/ACM.23000142 (So...

The current state of renal sympathetic denervation in hypertension
Arch Cardiol Mex. 2024 Feb 15. doi: 10.24875/ACM.23000191. Online ahead of print.ABSTRACTThis review provides an overview of the efficacy and safety of renal sympathetic denervation as a therapeutic approach for resistant hypertension. While the initial enthusiasm was sparked by the results of early clinical trials, it was dampened by the findings of the Symplicity HTN-3 study. However, recent advances in catheter technology and more refined patient selection criteria have yielded more promising results. Subsequent studies, such as SPYRAL HTN-OFF MED and RADIANCE II, demonstrated significant reductions in blood pressure, e...

Impact of diabetes on cardiovascular risk in patients with dyslipidemia
CONCLUSION: Having diabetes and dyslipidemia doubles the cardiovascular risk of patients. Little metabolic control was found in the population studied, which significantly increases complications at an early age and the economic burden on the health system and the families of patients, so it is necessary to rethink treatment strategies to improve metabolic control and with it the prognosis for the patient in the long term.PMID:38346327 | DOI:10.24875/ACM.23000042 (Source: Archivos de Cardiologia de Mexico)

Left ventricular hypertrophy: hypertensive or hypertrophic cardiomyopathy? What a dilemma! A case report
We present the case of a 46-year-old Afro-American man with poorly controlled hypertension who was found to have severe LVH, and who presented with symptoms of exertional angina during follow-up. We will review the clinical features that can help us in the differential diagnosis in this context.PMID:38330445 | DOI:10.24875/ACM.23000063 (Source: Archivos de Cardiologia de Mexico)

Phosphodiesterase 5 inhibitors for the treatment of heart failure: a systematic review and meta-analysis
CONCLUSIONS: Current data suggest that PDE-5 inhibitors therapy does not improve prognosis or quality of life among HF patients. Summary: International consensus statement on nomenclature and classification of the congenital bicuspid aortic valve and its aortopathy, for clinical, surgical, interventional and research purposes
Arch Cardiol Mex. 2024 Feb 7. doi: 10.24875/ACM.24000002. Online ahead of print.ABSTRACTThis consensus of nomenclature and classification for congenital bicuspid aortic valve and its aortopathy is evidence-based and intended for universal use by physicians (both pediatricians and adults), echocardiographers, advanced cardiovascular imaging specialists, interventional cardiologists, cardiovascular surgeons, pathologists, geneticists, and researchers spanning these areas of clinical and basic research. In addition, as long as new key and reference research is available, this international consensus may be subject to change b...

Increased risk of bleeding in patients with atrial fibrillation and chronic kidney disease on hemodialysis treated with anticoagulants. A four center serie
CONCLUSIONS: The results of this study suggest an increased risk of bleeding in patients with AF and CKD on hemodialysis receiving anticoagulation, without a decrease in the risk of thrombotic events or all-cause mortality.PMID:38325359 | DOI:10.24875/ACM.23000006 (Source: Archivos de Cardiologia de Mexico)

C-reactive protein, cardiovascular issues of an acute-phase protein: an update for theclinician
Arch Cardiol Mex. 2024 Feb 2. doi: 10.24875/ACM.23000032. Online ahead of print.ABSTRACTInflammation is an important pathogenic factor for the development of atherosclerotic cardiovascular disease. Currently, the most frequently used biomarker reflecting systemic inflammation is C-reactive protein (CRP), an acute-phase protein produced primarily by hepatocytes under the influence of interleukin-6, interleukin-1 beta, and tumor necrosis factor. Growing evidence from epidemiological studies has shown a robust association between elevated serum or plasma CRP concentrations and the incidence of a first cardiovascular adverse e...
