Practice Patterns and Barriers to Vascular Genetic Testing among Vascular Surgeons
Engaging patients living with or at risk for aortic dissection via the Aortic Dissection Collaborative, physician education in vascular genetics was identified as a research priority. We surveyed vascular surgeons to characterize practice patterns, motivations, and barriers regarding aortopathy genetic testing. (Source: Annals of Vascular Surgery)

Cutdown is Associated with Higher 30-day Unplanned Readmissions and Wound Complications Than Percutaneous Access for EVAR
A 2023 Cochrane review showed no difference in bleeding/wound infection complications, short-term mortality and aneurysm exclusion between the percutaneous and cut-down approach for femoral access in endovascular aortic aneurysm repair (EVAR). In contrast, single-center studies have shown bilateral cutdown resulting in higher readmission rates due to higher rates of groin wound infections. Whether 30-day readmission rates vary by type of access during EVAR procedures is unknown. The goal of this study was to ascertain which femoral access approach for EVAR is associated with the lowest risk of 30-day readmission. (Source: ...

Asymptomatic carotid stenosis.
Analysis of the optimal form of therapy to reduce stroke for patients with carotid artery stenosis has proven difficult over the last decade with the continuous introduction of new forms of medical and surgical endovascular treatments. The rationale in selecting the most appropriate therapy is based often on the results of prospective randomized studies performed in academic centers. The results of multicenter prospective randomized studies depend on inclusion criteria, and patients with lower income, low educational attainment, and not registered less fortunate people, who conceptually are at higher risk from ischemic str...

Addressing The Impact of Deep Venous Stenting on The Management of Venous Ulcer
Venous ulcers are a late and severe form of CVI and account for 70% of all etiologies that cause leg ulcers in the lower limb and they account for 20% of the 2.5 million cases complaining of chronic venous disease. Our study aims to investigate the effect of venous stenting of the deep veins on the healing of the venous ulcer. (Source: Annals of Vascular Surgery)

Bailout Distal Internal Carotid Artery Stenting After Carotid Endarterectomy: Indications, Technique, and Outcomes
Distal internal carotid artery (ICA) stenting may be employed as a bailout maneuver when an inadequate endpoint or clamp injury is encountered at the time of carotid endarterectomy (CEA) in a surgically inaccessible region of the distal ICA. We sought to characterize the indications, technique, and outcomes for this infrequently encountered clinical scenario. (Source: Annals of Vascular Surgery)

A Scoping Review of Decision Support Tools for Patients with Lower Extremity Arterial Disease: Towards Shared-Decision Making
In recent years, decision support tools in various fields of medicine have emerged to aid clinicians and patients in the process of shared-decision making. This scoping review aims to identify the existing decision support tools for selecting treatments in lower extremity arterial disease and to evaluate their effectiveness in facilitating shared-decision making. Common Criticisms and Rebuttals of The BEST-CLI Trial
Peripheral artery disease (PAD) affects more than 230 million patients world-wide1 and its prevalence is increasing due to the aging world population and the epidemic of diabetes and metabolic syndrome. 2 Approximately 11% of those with PAD progress to chronic limb threatening ischemia (CLTI), the most severe form of chronic PAD3 that presents with non-healing wounds or ischemic rest pain4. Without revascularization, the rate of limb loss among those with CLTI is approximately 22% per year.5 In addition, patients with CLTI have a 2-year mortality rate as high as 40% and the risk of death is nearly 3-fold among those with m...

Anticoagulation in patients with premature peripheral artery disease undergoing lower extremity revascularization
Premature peripheral artery disease (PAD), defined by lower extremity revascularization (LER) at age ≤ 50 years, is associated with poor major adverse limb events. The early onset of disease is thought to be influenced by genetic factors that regulate homeostasis of the vascular wall and coagulation. The aim of this study is to investigate the effect of anticoagulation as an adjunct to antiplatel et therapy on the outcomes of LER in patients with premature PAD. The Management of Aneurysmal Degeneration of Lower Extremity Cryopreserved Saphenous Vein Bypass Grafts
The use of cryopreserved saphenous veins (CSVs) for the treatment of lower extremity peripheral arterial disease is an attractive option when there is no available autogenous vein. Prior studies found CSVs are at risk for aneurysmal degeneration requiring reoperation. As the management of these complications and patient outcomes is not well described, the objective of this case series is to describe the open and endovascular management of degenerative CSVs at a tertiary community center. Glycaemic Variability as a Predictor of Graft Failure following Infra-Inguinal Bypass for Peripheral Arterial Disease. A Retrospective Cohort Study.
Glycaemic variability (GV), measured as the change in visit-to-visit glycated haemoglobin (HbA1c), increases the risk of multiple adverse outcomes. However, the impact of GV on graft patency following infra-inguinal bypass (IIB) is unknown. A retrospective cohort study was undertaken to assess the impact of GV on graft patency. Popliteal-to-distal extreme bypass in endovascular era
Chronic limb-threatening ischemia (CLTI) is characterized by rest pain and tissue loss, with an annual mortality rate of 20% and amputation rate of 40%, if not treated. Open bypass surgery is recommended in CLTI, depending on the availability of good quality venous material, outflow artery patency and surgical expertise. Aim of the study is to analyze primary patency, limb salvage and survival rate in patient undergoing popliteal-to-distal bypass. (Source: Annals of Vascular Surgery)
