Comparison of Non-Autologous Hemodialysis Grafts: Bovine Carotid Artery Versus Synthetic
An arteriovenous fistula is considered the ideal option for first-time hemodialysis arteriovenous access (AVA). For patients presenting with poor caliber upper extremity veins, is unclear which non-autologous conduit choice is superior. We aimed to compare outcomes between bovine carotid artery (BCA) and synthetic grafts for first-time upper extremity AVA. (Source: Annals of Vascular Surgery)

Indications for Urgent and Emergent Hemodialysis
The National Kidney Foundation states that hyperkalemia, metabolic acidosis, fluid overload, and uremic pericarditis are commonly accepted indications for acute renal replacement therapy. However, there is minimal literature supporting these guidelines. Furthermore, there is no data to date that addresses the timing and urgency of hemodialysis initiation. The placement of hemodialysis catheters after-hours is resource intensive and overburdens an already stressed system. The aim of this study is to identify which patients can safely wait until daytime hours to undergo catheter placement. (Source: Annals of Vascular Surgery)

Regional Anesthesia for Arteriovenous Fistula Creation Significantly Increases Venous Diameter
Preoperative vein and artery mapping with duplex ultrasound (DUS) is a standard practice to assess the size and depth of vessels being considered for use in arteriovenous (AV) access creation for hemodialysis. Data from the preoperative vein mapping study assists the surgeon to formulate an operative plan. Care is taken when selecting a site for AV access to maximize patency rates with the use of a single access configuration, provide more options for access sites and minimize the number of interventions patients undergo. Endovascular Salvage for Early Failure of Arteriovenous Fistulas
A significant portion of arteriovenous fistulas (AVF) (28-53%) fail before reaching maturation. Failure before successful cannulation, or within 3 months of usage is termed “early failure” while failure after this period is termed late failure. The two most common causes of early failure include accessory vein formation and stenosis. Both early and late failure lead to repeat operations and need for tunneled catheter placement which increase morbidity, mortality, a nd costs. While salvage of a mature AVF is commonplace in vascular surgery, salvage before initial cannulation of an AVF is a relatively uncommon procedure ...

Primary Inferior Vena Cava Leiomyosarcoma: A Rare Cause of Retroperitoneal Mass
Primary leiomyosarcoma of the inferior vena cava (IVC) is a rare, often aggressive, malignant tumor within the retroperitoneum. Due to low incidence, nonspecific clinical features, and variable imaging findings, delays in diagnosis and treatment are common. Limited evidence is available regarding optimal management, although surgical resection is the recommended primary treatment modality. Small studies have attempted to compare outcomes with IVC reconstruction versus no reconstruction, but definitive guidelines have yet to be established due to the lack of sufficient data. Stitch Granuloma in Below Knee Amputations with Myodesis
Vascular surgeons perform below knee amputations for nonviable, life threatening, limbs in patients with chronic limb ischemia secondary to peripheral arterial disease and diabetes mellitus. Myodesis is a technique utilized to secure muscle and tendon to bone in order to stabilize the residual limb. Myodesis has become a vital part of the rehabilitation process in amputees today as it maintains residual muscle function, improves efficacy of prosthetic use, and enhances the patient ’s ability to perform daily activities. Abdominal Aortic Aneurysm with Horseshoe Kidney
A Horseshoe Kidney (HSK) is a rare anatomic anomaly, with a prevalence of 0.12 percent, that poses significant challenges in the repair of concomitant abdominal aortic aneurysms (AAA). HSK requires operative planning with careful evaluation of not only the patient ’s comorbidities, but also the anatomic variations of HSK which include renal perfusion patterns and anomalous accessory renal arteries. Historically, HSK have been classified into five types based on the anatomy of the renal arteries which can be from the aorta, common iliac, hypogastric, or medi an sacral arteries. Image Guidance Techniques and Treatment Approach Optimization in the Management of Type-II Endoleak After Endovascular Aortic Aneurysm Repair
Over the past 3 decades endovascular aortic aneurysm repair emerged as the primary approach for abdominal aortic aneurysm management, however the occurrence of endoleak following endograft implantation imposes a high toll on patients and hospitals alike. The early diagnosis and appropriate treatment of endoleaks is associated with better outcomes, which calls for more advanced imaging and a standardized approach for endoleak diagnosis and management following endovascular aortic aneurysm repair. Image-guidance techniques and treatment approach optimization in the management of Type-II Endoleak after EVAR
Over the past three decades EVAR emerged as the primary approach for abdominal aortic aneurysm management, however the occurrence of endoleak following endograft implantation imposes a high toll on patients and hospitals alike. The early diagnosis and appropriate treatment of endoleaks is associated with better outcomes, which calls for more advanced imaging and a standardized approach for endoleak diagnosis and management following EVAR.Although conventional strategy with non-targeted deployment of coils and embolic material in the aneurysm sac is considered to be the standard approach in many hospitals, it may not prove ...

Perioperative Cerebrospinal Fluid Drain Placement Does Not Increase Venous Thromboembolism Risk after Thoracic and Fenestrated Endovascular Aortic Repair
Venous thromboembolism (VTE) incidence after thoracic and fenestrated endovascular aortic repair (TEVAR/FEVAR) is high (up to 6-7%) relative to other vascular procedures; however, the etiology for this discrepancy remains unknown. Notably, patients undergoing TEVAR/FEVAR commonly receive cerebrospinal fluid drains (CSFDs) for neuroprotection, requiring interruption of perioperative anticoagulation and prolonged immobility. We hypothesized that CSFDs are a risk factor for VTE after TEVAR/FEVAR. (Source: Annals of Vascular Surgery)
