Who manages fibromyalgia?
I'm sorry if this has been discussed elsewhere, but in y'alls practice setting, who manages fibromyalgia? I'm starting to get more and more of these patients with diffuse muscle and joint pain, especially now that I just started seeing medicaid, yay. Typically I will refer to PT and a pain psych consult for cbt (which >50% never go to, and I'm in a small city with no good psych so refer outside), maybe start gabapentin/lyrica/cymbalta. What do programs look for?? FAQ
I got a lot out of this forum when I was a medical student. So now that I'm on the other side I would like to give back. I am in the residency leadership at program X. c-section case
30 y/o 200lb Mallanpati #3 patient with high anxiety, fibromyalgia with multiple psych issues who has had a localized allergic reaction to lidocaine (says she had a raised red rash on arm from iv start with lidocaine). wants a general anesthetic for repeat c-section. (1st c-section was under general due to failed epidural- no anesthetic record available). Pt convinced to have spinal. When to turn a doc in?
I recently got a curbside on a patient who was on astronomical doses of OxyContin for fibromyalgia. Her PCP had been treating her for years with massive doses and it has come to the attention of several people. She also in the past apparently had some serious psych issues with threats of self-harm. I suggested to wean off and refer to addiction, but the PCP ignored this and has gone around regulatory roadblocks to continue this mis-management. My question is, at what point do you... 21 YO cf patient looking to move
I currently live in the greater kansas city area and see the KU CF team regularly. I am looking at the possibility of moving to Phoenix Arizona. I have a roommate lined up if I do but other than that I would be alone with the nearest family over 2 hours away. I have more than just my CF to contend with as well as typical CF problems with infections and viral and bacterial pneumonia within the past year. MY past PFT was only about 55% average.
