Why I'm leaving ACEP
This is a copy of the letter I sent ACEP: Dearest ACEP, I don’t know how to put this any other way, but I think we should break up. I know we have been together for many years and things were getting serious between us. I gave you thousands of dollars, and you gave me your fellowship pin. Although, it’s true I never got around to telling my parents about you. It’s not you, it’s me. You deserve someone more up to your standards. I have a dirty secret that I have never told you. I... De-Prescribing Thread
SDN Pain shamans, lets use this thread to post the good, bad and ugly about opioid deprescribing. We can share our success stories - and failures- and support each other. I look forward to learning new tricks, techniques, and ways to handle the difficult conversations that we have every day. I'll start- 45 yo F with fibromyalgia, PTSD, diffuse pain came to me from PCP on 140 MME, slurring her words, disheveled and disabled, unable to string together a coherent sentence. Three months later,... De-Prescribing Thread (Source: Student Doctor Network)

Who manages fibromyalgia?
I'm sorry if this has been discussed elsewhere, but in y'alls practice setting, who manages fibromyalgia? I'm starting to get more and more of these patients with diffuse muscle and joint pain, especially now that I just started seeing medicaid, yay. Typically I will refer to PT and a pain psych consult for cbt (which>50% never go to, and I'm in a small city with no good psych so refer outside), maybe start gabapentin/lyrica/cymbalta. There is no rheum in my town. I'll order the basic... Who manages fibromyalgia? (Source: Student Doctor Network)

What do programs look for?? FAQ
I got a lot out of this forum when I was a medical student. So now that I'm on the other side I would like to give back. I am in the residency leadership at program X. Every year, we review hundreds of applications, and here are the lessons learned. Keep in mind, much of this is my personal opinion and others might disagree. What do you look for in an application? I go straight to the SLOES and start at the bottom, where you are compared to your classmates in 2 categories. Take a look... What do programs look for?? FAQ (Source: Student Doctor Network)

Is fibromyalgia a psychosomatic illness?
Kind of an overly broad question that requires definitional clarification, but I think you get the picture. The purpose of the question is essentially to discuss whether individuals with fibromyalgia require predominantly psychiatric as opposed to medical treatment. (Source: Student Doctor Network)

When to turn a doc in?
I recently got a curbside on a patient who was on astronomical doses of OxyContin for fibromyalgia. Her PCP had been treating her for years with massive doses and it has come to the attention of several people. She also in the past apparently had some serious psych issues with threats of self-harm. I suggested to wean off and refer to addiction, but the PCP ignored this and has gone around regulatory roadblocks to continue this mis-management. My question is, at what point do you... 21 YO cf patient looking to move
I currently live in the greater kansas city area and see the KU CF team regularly. I am looking at the possibility of moving to Phoenix Arizona. I have a roommate lined up if I do but other than that I would be alone with the nearest family over 2 hours away. I have more than just my CF to contend with as well as typical CF problems with infections and viral and bacterial pneumonia within the past year. MY past PFT was only about 55% average. At the moment I am being tested for TB because of some symptoms I have been having including small portions of blood in my mucus from the lungs. Other problems i have include the rare...
