Question about anesthesia for peritonitis
I recently did this case of a patient with peritonitis, perforated bowel from a colonoscopy. Pt arrived shocked in ED, they gave him some fluids, he came to us with a bp of 100x60, lethargic. Attending decided to do induction with ketamine-S (people don't like the regular ketamine here)/fent/benzo, maintained with sevo/ intermitent ketamine boluses. It went downhill from there, he became hypotensive, had to give him high doses of norepi/epi. Afterwards, 22 issues at the ICU he stayed in the... GI procedures besides colonoscopy?
Completed IM rotation (M3) and in Cardiology there seems to be a lot of different procedures and tests when I read patient's notes. There's Angioplasty/Cath/Stent, CABG, Electrical Ablations, Valve replacements, etc. I've heard of a ton coming up. In GI, it seems like the only procedure I've seen is a sigmoidoscopy, EGD, or colonoscopy which can be therapeutic as they remove the polyp. I also don't see those going away because how can it when you have to go in and pluck thing something out.... GI procedures besides colonoscopy? (Source: Student Doctor Network)

How much **** do you see?
Serious question, I am interested in GI and was wondering when people see ****? For colonoscopies the intestines should be completely clean? Why are people complaining about seeing so much ****? The only time I can think is during emergency obstructions or something. Is that the only time you guys have to deal with smelly **** or is it a daily occurrence? Thanks for the insight. Virtual Colonoscopy
Does the neccesarily have to remain the domain of radiologist or will gastroenterologist be able to perform and interpert Virtual Colonoscopies? I don't see why not but there's alot I don't know so I was just curious as to who regulates who can interpert these types of tests. Seems like it would make more senes for the Gastroenterologist to do it so that if there is a poly they can remove it with a scope. Not sure what radiologist would do then..refer them to the gastroenterologist ? (Source: Student Doctor Network)
