80yo male with history of controlled a-fib presents with BRBPR in Aug 2021 so bad he "almost passed out." H and H 11 and 31. Colonoscopy 9/21 shows 5cm cecal mass. 10/21, hemicolectomy specimen shows pT3 4cm mass, margins all negative, MSI-, 0/24 LNs positive. He is sent to med onc for eval. Anyone in here doing the MIPS thing? Which quality measures are you doing? Practice Fusion only supplies a handful of them so I'm asking about colonoscopies and mammograms. Why don ’t we just start giving our own?
Dermpath didn’t ask pathology if it was ok to read skin cancers. Gastroenterologists didn’t ask surgeons if they could do colonoscopies. Neurologists didn’t ask if they could give TMZ. I just got into quite a big argument/debate in person with a few people and they said that being a doctor is the most overrated profession there is, and incredibly easy, no matter what specialty. They were being very serious. Anesthetic for EGD/Colonoscopy - low EF and/or severe AS?
What is your go to anesthetic for these sick GI patients? For these patients that have any of the above- severe pulm HTN, RV strain, severe AS, NSTEMi needing clearance for DAPT, 15% EF etc. A line vs NIBP? Topicalize and minimal anesthesia? Precedex versed? Doses? My go to has been A line then prop + ephedrine/phenylephrine. My colleagues never put in A lines for any GI pts and will use ketamine, precedex.
