"Abandonment" to help with adjacent emergency
Colleague came to me with this and I don't think it's quite cut and dry. Wanted to get the sense of the general sentiment of the populace. Anesthesiologist is staffing 3 offsite locations - 2 GI suites (ERCP room, colonoscopy room) & 1 IR room. 3 CRNAs. IR patient intubated and stable. ERCP intubated and stable. Colonoscopy for morbidly obese patient underway and going fine. As far as logistics, these NORA locations are about a 3-5 minute walk from the main ORs/central scheduling area... GI fellowship elective/away rotation in Japan?
I'm a current GI fellow and want to spend some time in Japan. I think that it'd be very interesting to see how medicine is practiced there and see how they do things differently in terms of management. Already, I can tell that people who train in Asia are much more talented at unsedated colonoscopies than in the US where we can rely on anesthesia. I have elective time, and I'm sure that I can convince my program to allow me to do an international elective in Japan if such a thing exists. I... Read more (Source: Student Doctor Network)

Matched at a place with low scope volume, how can I prep for private practice?
I matched at a place where fellows graduate with 600 total endoscopies including both EGD and colonoscopy. My goal is to go into private practice but unfortunately can't control the match algorithm. How can I make the best of the time I do have during fellowship to get additional volume/get good at endoscopy? Is there any other way for me to practice colonoscopies or get additional volume? Am I doomed to academics forever by ending up at a low volume place or will I be able to still escape... Read more (Source: Student Doctor Network)

Edible glitter?
Hi all, I moderate over in the pain forums and am scheduled Friday for my 3 year colonoscopy. Hoping to brighten the day of my GI doc and was wondering if edible glitter (maybe a teaspoon) would ruin the scope? It will be our third scope together and I see him regularly in my office (as a colleague) as we have a GI ASC down the hall from my procedure suite. Thanks. (Source: Student Doctor Network)

Colonoscopy Prep
Why do doctors order 4 L colonoscopy prep for some patients and then 8 L of colonoscopy prep for other patients? I was curious if there is a reasoning behind that. (Source: Student Doctor Network)

MSUCOM-EL vs. KCU-Jopling
I have been blessed with multiple D.O. acceptances and I’ve narrowed it down to these two schools! I would OOS state for both as I am a Florida resident. I don’t know exactly what I want to do in terms of specialty but I am definitely leaning towards hands-on specialties such as surgery, or G.I. (specifically Neurogastro b/c of doing procedures like colonoscopy/endoscopies). With this being said I wrote this sporadically tired after work. There may be jumps and incomplete thoughts so I... FDA approves 'SKOUT' - a new AI device to detect colon polyps
https://www.cancertherapyadvisor.com/home/cancer-topics/gastrointestinal-cancers/fda-clears-skout-to-detect-colorectal-polyps-in-real-time-during-colonoscopy/ Interesting article that leads to more questions: Does FDA approval mean gastroenterologists can bill more for using the device vs standard colonoscopy? If so, will insurance companies reimburse for it? And on the path end, does this mean receiving more or less colon biopsies if this becomes widely adopted? I could see... Landmark RCT on colonoscopy screening for colon cancer
https://www.nejm.org/doi/full/10.1056/NEJMoa2208375 “The trial’s primary analysis found that colonoscopy only cut colon cancer risk by roughly a fifth, far below past estimates of the test’s efficacy, and didn’t provide any significant reduction in colon cancer mortality. Gastroenterologists, including Bretthauer, reacted to the trial’s results with a mixture of shock, disappointment, and even some mild disbelief. “This is a landmark study. It’s the first randomized trial... Read more (Source: Student Doctor Network)

GI fast track and endoscopy competency
Hi I am a fourth year medical student applying to fast track positions. Do you think the 1 year of GI training is enough to become confident and well trained in EGD and colonoscopy? Do you think in that 1 year we would be able to achieve the 3 minute time to cecum without affecting our ADR? Have you had fast track colleagues who just don't match up to par clinically with people who go through the full 3 years of training, do these fast trackers eventually catch up clinically anyway? How... Read more (Source: Student Doctor Network)

Private practice volume
I’m wondering if there’s someone with experience in private practice who can comment on pay rates. I’m just entering the adult job market. I see that colonoscopy is 3.4 RVU and Medicare pays $34 per RVU. How can private practice afford to pay the rates I’m seeing, typically greater than 500k? Are people really doing 20-30 colonoscopies a day in private practice? Does private insurance pay more per RVU? I feel like I’m missing something. (Source: Student Doctor Network)

lymphocytic colitis on sertraline
have a patient who was started on zoloft 50 mg , was having acutely worsening diarrhea over 4-8 weeks, was dx with c. diff and then had a colonoscopy dx with lymphocytic colitis. GI Doctor believes it is related to sertraline. I found one articel report of this in the literature (dont have acceess to this) . Is this actually possible. I have never heard of this. Private Equity in Healthcare
Since it’s been discussed here before, here’s an informative article discussing private equity in healthcare. Betting on ‘Golden Age’ of colonoscopies, private equity invests in gastro docs An aging population in need of regular cancer screenings has driven private equity companies, seeking profits, to invest in many gastroenterology practices and set up aggressive billing practices. www.modernhealthcare.com (Source: Student Doctor Network)

Patient self-referral
Long story short, is it ethically / lawfully legal to refer patients to yourselves? For example, you are double board certified for internal medicine and GI. Let say you work in a primary care clinic affiliated with hospital from M-Th, and then Friday u work as a GI in your own private clinic. And so you refer all those pts >50 yo to yourself on Friday for colonoscopy. Of course this is just an example, and can be applied to any other specialties, for example IM and surgery. what about if... Read more (Source: Student Doctor Network)
