Dear colleagues, I would greatly appreciate your input on this situation. cT2N0 triple positive breast cancer. Mastectomy and axillary clearance after neoadjuvant chemotherapy shows pCR in the breast and sentinel node biopsy shows ITC. RT? Leave alone? Thanks (Source: Student Doctor Network)

Restarting research
Have been a little surprised about the lack of ethics conversations being had about this issue right now, so thought I would post to start one here. Curious to hear about people's thoughts on research in the present climate and how your respective institutions are handling things. We have largely been closed aside from studies with imminent life-saving health benefits (generally not psychiatry/psychology - think "We won't stop your experimental chemotherapy"). We're now in the process of... Restarting research (Source: Student Doctor Network)

pN0(i+), do you treat the nodes?
Dear friends, I have a 39 yo female diagnosed with a 2.3 cm IDC of the left Breast. she underwent a partial mastectomy with sentinel node sampling. it is a grade 3 IDC, Ki67 30%, 2 LN were sampled, both positive with isolated cells <2mm (i+). LVSI +. she received adjuvant chemotherapy. i know that according to Z0011, it is justified not to irradiate the nodes. What is the best practice now, considering her age and the grade? chemotherapy prescription
hi all, i am a french resident and i would like to know more about oncology in the USA and Canada. i know that french MD can't work in the USA but there are agreements between Canada and France. My question is about the organisation of oncology over there. In France med onc is a speciality, but they often treat cancers that are not in the field of an organ medical speciality. For instance, pulmonologist treat lung cancer and can prescribe all treatments like conventional chemotherapies or... chemotherapy prescription (Source: Student Doctor Network)

Appropriate Level of Aggressiveness in Hem/Onc?
I have recently been wondering whether I went into the right field, because I often don't feel comfortable subjecting patients to fourth or fifth line therapy when I feel they would be better served with hospice. I am training in a large cancer center where 90% of my teachers push patients to the brink of death, with many to die with chemotherapy in the last 2 weeks of life. Is this an academics thing, or all of oncology thing? Maybe I should do benign heme. (Source: Student Doctor Network)
