A Risk Calculator for Post-Operative Urinary Retention (POUR) Following Vaginal Pelvic Floor Surgery: Multivariable Prediction Modelling
CONCLUSIONS: This POUR risk calculator will allow physicians to counsel patients pre-operatively on their risk of developing POUR after vaginal pelvic surgery and help focus discussion around potential management options.PMID:35596931 | DOI:10.1111/1471-0528.17225 (Source: BJOG : An International Journal of Obstetrics and Gynaecology)

Younger women with more pronounced benefit from combination liposomal bupivacaine pudendal block during posterior colporrhaphy
To evaluate the effect of age on vaginal pain on patients undergoing posterior colporrhaphy and analyze pain and analgesic use after preoperative regional pudendal nerve block. We hypothesized that pain and analgesic use would be higher in the younger population and that a combination of 1.3% liposomal and 0.25% plain bupivacaine intervention group would have the most benefit in the young. Infected pelvic hematoma following vaginal hysterectomy with uterosacral ligament suspension for treatment of apical prolapse
CONCLUSION: Pelvic hematoma following VUSLS is common as opposed to infected pelvic hematoma which seldom occurs. Concomitant posterior colporrhaphy was associated with infection.PMID:35180514 | DOI:10.1016/j.ejogrb.2022.02.006 (Source: Reproductive Biology)

Bupivacaine Use After Posterior Colporrhaphy to Reduce Postoperative Pain: A Multicenter, Double-Blinded, Placebo-Controlled, Randomized Clinical Trial
Conclusions Use of bupivacaine for muscle block after posterior colporrhaphy does not reduce postoperative pain, opiate use, and time to first bowel movement, or increase void trial success. (Source: Journal of Pelvic Medicine and Surgery)

Obstructed defecation syndrome in the first week after pelvic reconstructive surgery
ConclusionUsing a validated disease-specific questionnaire to identify ODS, this complication was identified in 23.6% of patients in the week after pelvic reconstructive surgery. Preoperative ODS was a significant and important risk factor for this complication. (Source: International Urogynecology Journal and Pelvic Floor Dysfunction)

Association of Patient Race With Type of Pelvic Organ Prolapse Surgery Performed and Adverse Events
This study aimed to determine if there are differences in (1) surgical procedures performed for pelvic organ prolapse (POP) and (2) rates of adverse events between racial groups. Methods We conducted a retrospective cohort study of women 18 years and older who underwent POP surgery using the 2005–2015 American College of Surgeons National Surgical Quality Improvement Program database. Race was categorized as Black, White, Hispanic, and other minority. Pelvic organ prolapse procedures were organized into 4 groups: (1) hysterectomy without concurrent POP procedures, (2) vaginal wall repair(s) only without apical sus...

Preoperative pudendal block with liposomal and plain bupivacaine reduces pain associated with posterior colporrhaphy: a double-blinded, randomized controlled trial
Pelvic reconstructive surgery may cause significant postoperative pain especially with posterior colporrhaphy, contributing to a longer hospital stay and increased pain medication utilization. Regional blocks are being increasingly utilized in gynecologic surgery to improve postoperative pain and decrease opioid usage, yet preoperative pudendal blocks have not been used routinely during posterior colporrhaphy. Risk factors for recurrence of pelvic organ prolapse after vaginal surgery among Ugandan women: a prospective cohort study
ConclusionRecurrence of POP was common. Younger women, and those with postoperative vaginal cuff infection, were more likely to experience recurrent prolapse after vaginal repair. (Source: International Urogynecology Journal)

Outcomes of a fixed skeletonised mini mesh implant for pelvic organ prolapse repair with uterine preservation
J Obstet Gynaecol. 2021 Jun 25:1-4. doi: 10.1080/01443615.2021.1916808. Online ahead of print.ABSTRACTMesh repair of pelvic organ prolapse (POP) is complicated, causing erosions, postoperative pain and surgical failure. We hypothesised that reducing the mesh size and fixating it would result in significant cure rates and reduce complication rates. Here, we present the effectiveness of mini mesh implants in POP reconstruction. Sixty women who underwent repair of stage III and IV apical prolapse with cystocele or rectocele using skeletonised mesh implant Seratom PA MR MN® were evaluated. Anatomical outcomes were assessed us...
