Is there hemodynamic advantage for co-dominant coronary circulation ?
Co-dominant coronary  circulation is defined as , when  posterior crux of the heart receives twigs from both right and left system making this water shed area with advantage of twin innervation. They essentially supply  inferior and posterior aspect of both left and right ventricle including the posterior aspect of interventricular septum. Image courtesy modified from :http://www.meddean.luc.edu/lumen/meded/mech/cases/case1/image4.JPG Traditionally inferior and basal aspects of heart are perceived (wrong tough !) as less important  than anterior  surface of heart.Infero posterior MI can be extensive and cause significa...

Pharmaceutical Pricing – A Reminder of the Value Equation
We previously wrote about the actions Congress is taking to "combat rising prescription drug prices," and the rhetoric politicians of all stripes are using in an attempt to force public opinion on their side. HHS has announced a pharmaceutical pricing forum set for November 20, Congress has committees on both sides working toward a "solution," and Hillary Clinton has said that if she is elected president, she would "demand a stop to excessive profiteering and marketing" by the drug industry. Often left out those discussions includes the value of medications, PhRMA has recently released a forty-deck slideshow that addresse...

FFR with CT angiography
Brief Review Fractional flow reserve (FFR) is usually estimated invasively using a Doppler flow wire introduced into the coronary artery and measuring the pressure drop across the coronary stenosis during maximal hyperemia induced by adenosine. FFR has been shown to be of great significance while assessing the severity of borderline coronary stenosis in deciding the need for coronary angioplasty. Fractional flow reserve estimated by computerized tomographic angiography (FFRCT) is a novel technique of getting similar information non invasively. FFRCT has been shown to be superior to conventional CT coronary angiography and ...

An episode of low-value care delivered to my father
My father recently visited his internist with complaints of vague chest symptoms and was referred to a cardiologist who recommended coronary CT, ultrasonography, and angiography.  A cardiac workup would seem to be a reasonable course of action given that he has a history of coronary artery disease (CAD) with prior angioplasty. However, upon further exploration, I felt that his chest symptoms were due to stress and were not of cardiac origin.  He exercises regularly without symptom provocation.  A few months ago our family went on a cruise, and my father would walk on the treadmill for hour-long sessions to pass time whi...

Anterorlateral STEMI? Old Anterior MI? But cath shows RCA thrombotic stenosis.
This case is from a frequent contributor, Brooks Walsh.  With additions and edits by me.A 68 year-old man had been having chest discomforts intermittently for the past 2 weeks. About 18 hours prior to presentation, his pain began to worsen. He developed nausea, and 911 was called after he vomited once. EMS obtained an ECG:There is subtle ST depression in II, III, and aVF. There is ST elevation in V2 – V5, with Q waves in V2 – V4.  This is all but diagnostic of anterior MI.Is it acute? subacute? Old MI with persistent ST elevation?EMS requested cath lab activation. The patient was given nitroglycerin 3 times, ...

Not All News Needs to Be Gloomy
One of the joys of working on the subject of human progress is learning about the many ways in which the world is becoming a better place. Keeping up with all of the scientific and technological breakthroughs is, of course, impossible. And so, here is a sliver of the good news that caught my eye last week. We are one step closer to solving the problem of organ shortages Harvesting organs from animals may provide the solution to the shortfall of human donors. However, two technical obstacles need to be overcome before animal-human transplants can become a medical reality. One is that human immune systems often reject foreig...

Factors predicting stroke after CABG – Cardiology MCQ
Factors predicting stroke after CABG: a) Presence of intracerebral and extracerebral atherosclerotic disease b) Demonstration of previous stroke by imaging c) Atheromatous disease of the aorta d) All of the above Correct answer: d) All of the above Aortic plaques have a high chance of embolization while cannulating the aorta for cardiopulmonary bypass. Avoiding cardiopulmonary bypass with off pump CABG (OPCAB) may have an edge over conventional on pump CABG in this situation. Recannalised IRA : When God’s hand is natural aspiration catheter . . . better show restraint .
“Oh , it’s a well recannalised IRA  and its flowing TIMI 3  as well.  Now, what shall we do sir” ?, An apparently worried senior resident queried after a second look at the images from a 8 hour old STMEI .Why you sound unhappy  man ?  As if recanalisation is an untoward event” ! I teased my resident ! and went on to ask . . . What we mean by recannalised  IRA ? (Recan-IRA) It is akin to natural or pharmacological angioplasty (or combination of the two ) It can be complete or incomplete from the IRA perspective. It can either result in partial or fully salvaged myocardium. It should be ...

Think twice before having carotid artery surgery
Hundreds of thousands of people have undergone surgery or stents to “fix” blockages in their carotid arteries. (The left and right carotids are the main arteries to the brain.) Most of the these people (about 90%) reported no complaints. We say they are asymptomatic. The blockages were discovered on exam or by ultrasound of the neck. The idea is that major blockages can progress to full blockages and cause stroke. Since it’s best to prevent stroke, patients with blockages get referred to surgeons and surgeons operate. The fix entails cleaning out the blockage, which we call endarterectomy. More recently, ...
