Usually it is the left iliac vein which is compressed by the right iliac artery. The compression increases the risk of deep vein thrombosis. IABP in action
IABP – monitor screenshot Green tracing: ECG. White tracing: Respiration. Red tracing: Intra arterial blood pressure. Yellow tracing: Pulse oximetry. Displays on right side, from top to bottom: Heart rate, respiratory rate, blood pressure, oxygen saturation (SPO2). Intra aortic balloon pump (IABP) or intra aortic balloon counterpulsation is used to augment the cardiac output and reduce the afterload to the heart. Classic indication for IABP is cardiogenic shock. It can be either in the setting of acute myocardial infarction, cardiac surgery or in a person undergoing high risk percutaneous coronary intervention. The I...

Critical management issues in CAD : OMG ! . . where is OMT ?
One of my fellows gave a discharge summary  for a 62 year old patient with stable diabetic  CAD  who had Triple vessel disease with a final advice reading as CABG / PCI/or OMT . There was a near fury over his angiogram report in the cath meet. How can be be  eligible for all the three Intervention at the same time ?.(PCI -Percutaneous coroanry Inervention ,CABG-Coroanry artery by-pass graft, OMT-Optimal medical therapy ) The lesion in question was , Triple vessel disease(Non critical LAD) and significant LCX and again a non critical RCA .Syntax was less than 22 for sure , however the patient  had class 2 angina (now r...

Value Pricing For Drugs: Whose Value, What Price?
It is hard to read a newspaper these days without coming across a story about the high and ever increasing cost of drugs. The Wall Street Journal named drug prices the top health story of 2015. Stories about drug prices fall into two general categories. The first are stories about generic and other manufacturers who are not focused on innovation (like Turing Pharmaceuticals and Valeant) but who acquire generic drugs and increase the price dramatically without adding value while also making it impossible for competitors to manufacture the drug by controlling the distribution. While this problem is real, it is not the focus ...

"Shark fin" ECG in I, aVL, V4 and V5. Which artery? Hint: patient is in shock and was put on ECMO
This was contributed by Rohin Francis (Twitter: @MedCrisis), a cardiologist from England and FOAM enthusiast.CaseA 55 year old lady initially presented to hospital with an acutely ischaemic arm. An embolic occlusion of her brachial artery was diagnosed by CT and treated with anticoagulation. The following day she developed sudden severe chest pain. This ECG was obtained:Sinus rhythm. The rather alarming appearance of the QRST may be mistaken for a broad complex QRS but,  in fact, her QRS complex can be clearly seen in V2, V3 and II and is narrow. What has manifested as triangular complexes is actually huge ST segment...

A Pathognomonic ECG
This was sent to me by Mauro Cassazza from Italy:What do you see?AnalysisIt is tempting to think that the waves before the QRS are large P-waves, but in fact one can discern P-waves within these large waves (see V2 in particular). Thus, these are very late T-waves.   Thus the QT interval is very long. I measure the QT interval at about 520 ms.  Since the RR interval is about 560 ms, then the Bazett-corrected QTc is about 690 ms (very long).  What part of the QT interval is long?   Is the T-wave itself very wide?  Is the time from T-wave peak to T-wave end (T-peak to T-end, or TpTe, long)?...

In Defense of Small Data
By ANISH KOKA, MD I read with interest a recent editorial that opined on the poor evidence for screening in cancer trials. The evidence was judged poor because apparently no screening trial has demonstrated a clear reduction in all-cause mortality, only disease-specific mortality.  One example discussed in the analysis reviews the data for colon cancer screening and notes that, while there were a statistically significant lower number of deaths related to colon cancer in the screened group, the total mortality in the two groups was no different.  The authors posit that the study is either underpowered for total mortali...

40 Years Ago, There Was a Poster at the American Heart Association Meeting…. 0
Today being TBT (Throw Back Thursday) on Twitter, Dr. Jordan Safirstein (@CardiacConsult) posted a photo this morning of a poster that appeared 40 years ago at the 1976 American Heart Association meeting. It’s a meaningful poster, since it was the first public presentation of the concept and initial studies of something called coronary angioplasty. Check out my video clip.
