Splenic abscess: a rare adverse event of a benign penetrating gastric ulcer

A 54-year-old woman taking long-term nonsteroidal anti-inflammatory drugs presented with coffee ground emesis and melena. She was tachycardic; the physical examination was otherwise unremarkable. Laboratory evaluation showed a nadir hemoglobin of 7.8 g/dL from a baseline of 11.0 g/dL. EGD was performed, which revealed a large, 2-cm, clean-based ulcer with a fibrotic base in the gastric fundus (A). Biopsy specimens for Helicobacter pylori showed negative results. The patient experienced no recurrent bleeding during hospitalization and was treated with twice-daily doses of a proton pump inhibitor for 8 weeks.
