Massive hydronephrosis, countless lithiasis and upper urinary tract carcinoma

A 51-year-old otherwise healthy man presented to the emergency department with a 6-month history of weight loss, asthenia, lumbar pain and hematuria. Urinalysis was notable for hematuria without significant pyuria. Hemoglobin was 10.3 g/dL and serum creatinine was 1.2 mg/dL. The remainder of the complete blood count, metabolic panel, and coagulation parameters were normal. An abdominal x-ray was done and based on the findings of this a CT scan was performed (Figure 2).
