First-line immune checkpoint inhibitors in advanced or metastatic renal cell carcinoma with sarcomatoid features

Sarcomatoid features in metastasized renal cell carcinoma (mRCC) were historically described as a histopathologic pattern consisting of an apparent pleomorphic mixture of malignant elements arising from the connective tissue, smooth muscle tissue or other tissues[1,2]. More recently, it has been characterized by the presence of malignant spindle cells that usually express markers of both epithelial and stromal differentiation[3] (Image 1). Any presence of sarcomatoid component is enough to determine the tumor as having sarcomatoid features[4,5].
