Unilateral polycystic kidney disease

A 30-year-old man was diagnosed with polycystic kidney disease (PKD) at the age of 14 when he was found to be hypertensive. Initial abdominal imaging showed markedly enlarged kidney cysts on the left kidney and a solitary cyst on the right kidney, without hepatic cysts. Neither his father nor his mother had kidney cysts on ultrasound. He developed worsening abdominal fullness, left flank discomfort, and back pain. Repeat abdomen magnetic resonance imaging (Figure  1) showed that the total kidney volume was 7531 ml, with 7160 ml attributed to the left kidney.
