Inappropriate low dosing of direct oral anticoagulants in older patients with non-valvular atrial fibrillation: Impact on plasma drug levels

Direct oral anticoagulants (DOACs) are increasingly prescribed in older patients with non-valvular atrial fibrillation (NVAF). They provide higher convenience than vitamin K antagonists (VKA), with an improved efficacy and a reduced risk of intracranial bleeding [1]. Dose reduction criteria are clearly established for each DOAC, depending on renal function, weight, age or concomitant medications. However, many observational studies have reported that DOACs were frequently prescribed at an off-label dose, especially in older patients [2].
