African Americans (AA) are more frequently diagnosed with prostate cancer and when prostate cancer is identified it tends to be diagnosed at earlier ages, have more poor risk features, and more frequently result in metastases or death. These figures are known by all urologists, it drives our thinking and understanding about prostate cancer in AA men. Too often these figures along with a handful of poorly performed small retrospective studies and a few “what if” scenarios extrapolating radical prostatectomy pathology in potential candidates for active surveillance lead physicians to recommend against active surveillance for AA men with low risk prostate cancer.
