Splenic abscess and association with melioidosis in an endemic region: A retrospective review of 39 cases in Kapit, Sarawak, Malaysian Borneo

Background: Splenic abscess is an uncommon condition with an estimated incidence rate of 0.05 –0.7% in autopsy studies. Causative microorganisms can be very diverse. However, in areas where melioidosis is endemic, Burkholderia pseudomallei is the most frequent cause of splenic abscess.

We report the first case of native gestational and neonatal melioidosis in the Western hemisphere. The isolated strains in the mother and newborn were confirmed by whole-genome sequencing and identified as a novel sequence type ST1748. The comparison of both genomes revealed a nucleotide similarity of 100%. Melioidosis should be considered within the differential diagnosis of febrile illness or pneumonia in pregnant women and newborns from endemic areas of the Americas. In this study, multilocus variable-number tandem repeat analysis (MLVA), and multilocus sequence typing (MLST) were applied to examine the epidemiological relatedness and population structure of 166 B. pseudomallei isolates obtained during 2002–2014 in Hainan, China.
