Syndromic association of aortopulmonary window in an adult: Hitherto unreported occurrence deciphered using cardiovascular CT

A 27-year-old male, a known case of Axenfeld-Reiger syndrome with congenital glaucoma, was being evaluated for ophthalmic surgery. For the past 6 months, he complained of dyspnea on exertion and palpitations. Prior to this, there was no history of any cardiac symptoms. Echocardiography revealed severe pulmonary artery hypertension with right ventricle hypertrophy and normal biventricular function. Cardiac CT angiography revealed presence of defect in aortopulmonary septum, approximately 2 cm above the sinotubular junction, suggestive of aortopulmonary window (APW).
