CONCLUSION: The complexity of the diagnosis of pulmonary aspergillosis is related to its multiple types with sometimes unclear distinctions. Any type of pulmonary aspergillosis can be observed in patients with COPD, depending on associated risks factors. Conclusion: We conclude that basophil and lymphocyte activation tests could enhance the diagnosis of allergic mycosis, compared to usual humoral markers. Further studies with larger cohorts and addressing both mold extracts and mold relevant molecules are needed in order to confirm and extend the application of this personalized medicine approach.

Conclusion: In patients with suspicion of pulmonary Aspergillosis, BAL galactomannan level was found to be a better marker than fungal smear/culture. While microbiological and histopathological evidences are gold standard to diagnose fungal infections, our study shows that in patients with negative fungal smear or culture, BAL Galactomannan can be conclusive for diagnosis.
