A 78-year-old woman with heart failure

A 78-year-old woman was admitted because of heart failure. She was known to have nonischemic dilated cardiomyopathy. During the past 2 weeks, she developed severe shortness of breath. The people around her also noticed memory loss and confusion in time and place, suggesting that she was becoming demented. On admission, her daily medication consisted of a β-blocking agent, an angiotensin-converting enzyme inhibitor, a diuretic, and 1/8 mg of digoxin.
