Drug-Induced Anaphylaxis in a Vietnamese Pharmacovigilance Database: Trends and Specific Signals from a Disproportionality Analysis

AbstractIntroductionDespite the numerous studies investigating drug-induced anaphylaxis (DIA), understanding and quantitative data analysis in developing countries remain limited. The aim of our study is to describe and quantify DIA using the National Pharmacovigilance Database of Vietnam (NPDV).MethodsSpontaneous reporting of adverse drug reactions (ADRs) recorded between 2010 and 2016 were retrospectively analysed to identify DIA reports. The trend and characteristics of DIA cases were described. Multivariate disproportionality analysis was used for signal generation.ResultsOverall, 4873 DIA cases (13.2% of total ADRs) were recorded in the NPDV, 111 of which resulted in death (82% of total ADR-induced deaths) over a 7-year period. ConclusionNo patient experienced adverse events directly related to gadoterate meglumine. Only two adverse events were reported to have occurred in the initial 2  h after the exam, while the rest were reported on the 24-h follow-up call. The higher reported rate of adverse events in this study may be related to concomitant sedation/anesthesia as well as to overreporting from parents on the 24-h follow-up questionnaire. The study confirms a good safety profi le for gadoterate meglumine in this very sensitive population.

CONCLUSIONS: This secondary analysis demonstrates that the incidence of severe critical incidence was lower in Italy in comparison to that reported for Europe. Abstract We here report that a 71-year-old Japanese woman with a history of anaphylaxis induced by muscle relaxants had local anesthetic systemic toxicity (LAST) following an abdominal surgery under general anesthesia with combined spinal-epidural anesthesia without muscle relaxants. The rate of caesarean delivery (CD) has tripled in the last three decades.1 Most CD are performed under regional anaesthesia because of its various advantages: better parental experience associated with the delivery; avoidance of exposure to general anaesthetic agents that have a risk of causing anaphylaxis and uterine atony; avoidance of intraoperative awareness; and reduced mortality.2 Importantly, regional anaesthesia avoids or minimizes the risks of airway management and its associated complications such as failed intubation, oesophageal intubation and aspiration.
