Team based care

Over the past few years, heart failure clinics have evolved into a strong example of team work but it was not always this way. The traditional clinic, where the nurse took the vital signs and escorted patients into the exam room is rapidly disappearing in the heart failure arena. Many studies have shown the value of nurses in dealing with this patient population and especially in the area of preventing readmissions. By letting nurses do what they do best, everyone benefits. Education of patients is our forte and this has shown to be one of the main ways to encourage self-care and improve outcomes.
