Chemotherapy for MCL Debate: The Positive Side

Mantle cell lymphoma (MCL) is a heterogeneous disease that cannot be treated with uniform regimens. Aggressive MCL (blastoid, pleomorphic, bulky tumor mass> 7 cm, high Ki-67, TP53 mutation/deletion, c-myc and NOTCH positivity) cannot be treated with single agents alone and needs to be treated with combination chemotherapy, combinatory biological agents, cellular therapy, or “mini” stem cell transplantation. In daily routine practice, we treat young, fit MCL patients and elderly MCL patients differently.
