CONCLUSIONS: HFNC should be considered for pediatric ED patients with respiratory distress not requiring immediate endotracheal intubation; prospective, pediatric ED-specific trials are needed to better determine responsive patient populations, ideal HFNC settings, and comparative efficacy vs. other respiratory support modalities.
