Conclusions:Proposed mechanisms for SDH-associated insular infarction include venous hypertension and traumatic arterial vasospasm. SDH-associated insular infarction is a marker of greater disease severity. Advanced imaging findings may be an important biomarker of disease severity and may influence complex management decisions.
