Reduction in 30 Day Readmission of Stroke Patients from Acute Care Centers with the Involvement of Medical Hospitalists (P1.214)
Conclusions: In the absence of a medical hospitalist, stroke patients are predominantly readmitted for infectious etiology. Readmission complications are decreased among those discharged from a primary hospitalist service. We conclude that involving medical hospitalists in the care of stroke patients during their initial admission optimizes discharge condition; thus reducing the risk of complications and decreasing frequency of re-hospitalizations.Disclosure: Dr. Hua has nothing to disclose.
